This review assessed the real-world evidence of the management of macular oedema secondary to branch retinal vein occlusion (BRVO). A meta-analysis of 2530 eyes from 48 real-world studies of therapies for macular oedema secondary to BRVO was conducted. Baseline characteristics, visual, anatomical and safety outcomes were recorded. The weighted mean and weighted estimates from random-effects models were calculated for visual acuity (VA) and central subfield thickness (CST) changes at 6, 12 and 24 months. Primary outcome was change in VA (logMAR letters) at 12 months. Study quality was assessed using the quality appraisal checklist for case series developed by Institute of Health Economics. The mean baseline VA for the pooled data was 54.0 (51.5, 56.5) letters and the mean baseline CST was 501.3 (483.5, 519.1) µm. The random-effects estimate for mean (95% CI) change in VA was 14.6 (12.5, 16.7) letters at 12 months ( n = 1727). The random-effects estimate for mean (95% CI) change in CST was −181.7 (−230.7, −132.7) µm at 12 months ( n = 1325). The quality of studies varied considerably. Ocular and systemic adverse events were discussed in 79% and 42% of treatment arms respectively, with possible under-reporting. Visual and anatomical gains achieved in the real-world for anti-VEGF therapy were not as impressive as seminal RCTs, possibly due to reduced injection frequency in the real world and differences in baseline characteristics. There is an urgent need for consensus on the minimum efficacy, treatment burden and safety data to collect to strengthen the real-world evidence base.

Background: The management of macular edema and ocular neovascularization is changing and includes a new group of drugs called anti-vascular endothelial growth factor (anti-VEGF). Intra-vitreal injection of anti-VEGF agents has become the new standard of care for macular edema. However, data on their real world effectiveness and safety of these drugs in African eye care settings are very scarce. Objective: To assess the visual outcome of intravitreal Avastin (IVA) injection at University of Gondar hospital tertiary eye-care and training center. Methods: A retrospective analysis of medical records of patients who received IVA at the center was done. The main outcome measure was visual acuity (VA). Results: The study included 37 eyes of 34 study participants with macular edema secondary to diabetic retinopathy, retinal vein occlusions, and neovascular age related macular degeneration (AMD). Mean VA improved from 6/60 (approximate 35 ETDRS letters) at baseline to 6/24 (approximate 55 ETDRS letters) at 2 months follow-up (p=0.0045) and this improvement was maintained at 6 months of follow up. This happened after mean injection of 2.5 times per eye over 6 months period. No major ocular or systemic treatment related adverse events were observed. Conclusion: Patients who received IVA as initial therapy for macular edema from diabetic retinopathy, retinal vein occlusions, and neovascular AMD has a significant mean VA improvement which was maintained up to 6 months. Short term results show that IVA is effective and safe.

Introduction: The most common cause of vision loss in cases of Retinal vein occlusion (RVO) is due to macular edema. This study was conducted to examine the effect of intravitreal bevacizumab (IVB) in the treatment of macular edema secondary to RVO. Materials and methods: The authors conducted a retrospective study of 94 eyes (N) of 92 patients with macular edema associated with decreased visual acuity secondary to RVO who were treated with IVB. Patients received IVB at baseline, 1 month and 2 months. At each follow up patients were evaluated and re-injected if necessary. Results: The mean age of the patients was 56.6 ±11.51 years. The average number of injections per eye was 2.1 ± 0.87. The baseline median central macular thickness (CMT) and best-corrected visual acuity (BCVA) in LogMAR was 465.00μm (Min 254μm, Max 1218μm) and 1.00 (Min 0.30, Max 2.28), respectively. The median CMT at one month following first, second and third dose of IVB was 258μm (N=94, Z= -7.64, p <0.001), 261μm (N=63, Z= -0.17, p=0.86), and 292μm (N=41, Z= -0.21, p= 0.83), respectively. The median LogMAR BCVA at one month following first, second and third dose of IVB was 0.60 (N=94, Z= -5.70, p < 0.001), 0.60 (N=63, Z= -1.69, p=0.09), and 0.60 (N=41, Z= -0.03, p=0.97), respectively. Pre-operative BCVA was the best predictor of the final visual outcome after IVB in cases of RVO. None of the patients developed any serious ocular or systemic complications due to IVB. Conclusion: IVB is an effective and safe treatment for macular edema associated with decreased visual acuity secondary to RVO. The most beneficial effect of IVB is seen at one month after the first dose. The efficacy of subsequent doses could not be established in this study
