Bone marrow lesions (BMLs) or using older terminology 'Bone marrow edema' is characterised by excessive water signals in the marrow space on magnetic resonance imaging or ultrasound; BMLs constitute a central component of a wide variety of inflammatory and non-inflammatory rheumatologic conditions affecting the musculoskeletal system: BMLs are not only considered significant sources of pain but also linked to increased disease activity in many musculoskeletal conditions (for example, osteoarthritis, rheumatoid arthritis). The purpose of this review is to summarise current knowledge about the treatment of BMLs, with an emphasis on the clinical and histological features of this entity in inflammatory and non-inflammatory disease. We also try to pair this hypothesis with the apparent beneficial effects of various treatment regimens, mainly within the group of bone antiresorptive drugs (calcitonin, bisphosphonates) on symptoms associated with BMLs.

Treatment with acetylsalicylic acid prevents short to mid-term radiographic progression of nontraumatic osteonecrosis of the femoral head: a pilot study Background: Nontraumatic osteonecrosis of the femoral head (ONFH) is a progressive disease in young adults producing substantial morbidity and frequently resulting in total hip arthroplasty. Although hip-preserving surgical procedures represent the current mainstay of treatment for early disease, medical therapies targeting specific pathways in the ONFH pathogenesis could help prevent disease progression while producing less morbidity. Acetylsalicylic acid (ASA) is a promising alternative to other therapies for ONFH owing to its anti-inflammatory and antithrombotic mechanisms of action and its relatively benign side effect profile. Methods:We followed a prospective cohort of 10 patients (12 hips) with precollapse ONFH who were given ASA to prevent disease progression. Their outcomes were compared with those of a historic control group taken from the literature.Results: Progression occurred in 1 of 12 (8%) patients taking ASA compared with 30 of 45 (66.6%) controls (p = 0.002) at a mean follow-up of 3.7 years. Patients taking ASA also tended to exhibit decreased femoral head involvement at the end of therapy. Conclusion:This hypothesis-generating study leads us to believe that ASA may be a simple and effective treatment option for delaying disease progression in patients with early-stage ONFH.Contexte : L'ostéonécrose non traumatique de la tête fémorale (ONTF) est une maladie progressive qui affecte les adultes jeunes, s'accompagne d'une morbidité substantielle et mène souvent à une arthroplastie totale de la hanche. Même si les interventions chirurgicales visant à préserver la hanche représentent la pierre angulaire actuelle du traitement pour la maladie au stade précoce, les traitements médica-menteux qui ciblent les voies spécifiques de la pathogenèse de l'ONTF pourraient contribuer à prévenir la progression de la maladie tout en atténuant la morbidité. L'acide acétylsalicylique (AAS) est une solution de rechange prometteuse aux autres traitements indiqués pour l'ONTF en raison de ses propriétés anti-inflammatoires et antithrombotiques et de son profil d'innocuité relativement bénin.Méthodes : Nous avons suivi une cohorte prospective de 10 patients (12 hanches) présentant une ONTF au stade précollapsus qui ont reçu de l'AAS pour prévenir la progression de la maladie. Leurs résultats ont été comparés à ceux d'un groupe témoin historique de patients décrits dans la littérature.Résultats : La progression a affecté 1 patient sur 12 (8 %) traités par AAS, contre 30 témoins sur 45 (66,6 %) (p = 0,002) après un suivi moyen de 3,7 ans. Konservatif cihaz veya alçı tedavileri ve osteotomiler, sıkça kullanılan tedavi metodlarıdır. İki adet çok merkezli, ileriye dönük çalışmada, tedavi metodlarının etkinliği kapsanma açısından değerlendirilmiştir. [4][5][6] Her iki çalışmada da, küresel femur başı elde etmek hedef olarak alınmıştır. Hastalık başlangıcı altı yaşından önce olan çocuklarda konservatif tedavi yöntemleri daha etkili bulunurken, altı yaşından büyük çocuklar-da osteotomi uygulamaları daha başarılı görülmüştür. Buna rağmen, başarılı tedavi sonuçları %30-70 arasın-da değişmektedir. Hastalar ve tedavi metodları arasın-da geniş farklılıkların olması, son yıllarda patobiyolojiye yönelik tedavi seçenekleri üzerindeki araştırmaları arttırmıştır. Femur başı deformitesini engellemek bu sayede mümkün olabilir. Femur başı deformitesinin patofizyolojisi üzerinde birçok deneysel çalışma yapıl-mıştır. Bu çalışmalarda, osteoklastik aktivitenin, nekroz gelişimi ve sonrasında çökme gelişmesinde etkili olduğu ortaya konulmuştur. Bu etiyoljik nedene yönelik olarak da, deneysel çalışmalarda osteoklast aktivitesini baskılamak için bifosfonatlar, kemik yapımını uyarmak için damarsal endoteliyal büyüme faktörü, kemik L egg-Calvé-Pertes hastalığı (LCPD), nedeni tam olarak bilinmeyen, juvenil yaşta görülen femur başının osteonekrozudur. Kalıcı femur başı deformitesine ve erken dönemde osteoartrit gelişmesine neden olabilir. Çocukluk döneminde 2-14 yaşları arasında femur başı osteonekrozu nedenleri arasında en sık görülenidir; sıklığı 100000 de 5,1-16,9 arasında değişmektedir. Yüz yıldan fazla süredir bilinmesine rağ-men, elimizde etiyoloji, patofizyoloji, tedavi ve doğal seyriyle ilgili çok az bilgi bulunmaktadır. [1][2][3] Etiyolojisi bilinmediği için, günlük tedavi metodları doğal seyrini değiştirmeye yönelik uygulanan metodlardır. Tedavi prensipleri, genellikle femur başının asetabulum tarafından kapsanmasına ve kalça eklemi hareket genişliğinin korunmasına yöneliktir. Bu prensipler uygulanırsa, nekroz nedeniyle yumuşamış olan femur başının küreselliğini korunması mümkün olabilir. Bu amaçla birçok tedavi yöntemi tanımlanmış ve kullanılıyor olmasına rağmen, hiçbiri tam olarak femur başının örtünmesini ve küreselliğinin korunmasını sağ-lamamaktadır. Ayrıca, bu tedavi metodları öngörülebi-lir sonuçlar vermemektedir. Legg-Calvé-Perthes hastalığı, 100 yılı aşkın süredir bilinmesine rağmen, elimizde etiyoloji, patofizyoloji ve doğal seyri ile ilgili çok az bilgi bulunmaktadır.
