Liposomes are vesicles consisting of spherical phospholipid bi-layers with specific properties making them useful for topical application of drugs. Liposome research has expanded considerably over the last 30 years and nowadays, it is possible to construct a wide range of liposomes varying in size, phospholipids composition and surface characteristics to suit the specific application for which they are intended. In dermatology, the topical application of liposomes has proven to be of therapeutic value. Liposomes can be used as carriers for hydrophilic as well as lipophilic therapeutic agents because of their amphipathic character. They may improve stabilization of instable drugs by encapsulating them and serve as penetration enhancers facilitating the transport of compounds that otherwise cannot penetrate the skin. Liposomes help in reducing skin irritation by sustaining the release of drugs and by hydration of the epidermis. They also have the potential to target drugs into the pilosebaceous structures and hence they have an additional advantage for treatment of hair follicle-associated disorders. Clinical data indicate that 5-ALA encapsulated in liposomes improves the quality of Fluorescence Diagnosis by ALA-induced Porphyrins (FD) and optimizes the results of Photodynamic Therapy (PDT).

Superficial vein thrombosis (SVT) is a condition of clotting of superficial veins associated with minimal inflammatory components, while superficial thrombophlebitis (STP) is defined as a condition of minimal thrombosis of superficial veins associated with inflammation or infection. In UK it is estimated to have 400 cases of SVT in every 100 000 person per year in comparison to 125 000 case annually in the United States. SVT and STP tend to occur more in old ages because of increased incidence of varicose veins, with a tendency toward working population. Women are affected more in the UK, but the San-Valantino study showed a similar rate. Obese low mobility people, pregnant ladies, females on contraceptives and hormone replacement therapy (HRT) are more affected by the two conditions. Conventional treatment of both conditions is conservative and usually includes pain killers, compression, anti-inflammatory creams and elevation. Anticoagulation is indicated in some cases, whether systemic or topical. SVT and STP are better prevented than treated, and some simple steps help to achieve that goal. Removal of intravenous (IV) catheters after maximum 48 hours of usage and avoidance of small veins while puncturing are some but not all. Low molecular weight heparin (LMWH) and nitroglycerin patches distal to peripheral lines can also prevent SVT and STP. Exercise is recommended while total bed rest is not in such cases. Antibiotics are usually conserved for cases with proved source of infection. Our main concern in this paper is to review the literature regarding the use of topical heparins in SVT and STP. Although it was mentioned in many trials and expert opinions, the use of topical heparin is not yet standardized and its use is limited to some centers only. It still lacks the enough evidence because of deficiency in large pharmaceutical investments in this field, yet it has shown many beneficial effects in clinical practice. Scientifically speaking, three main effects of heparin on skin can be observed; a)the anti-coagulation effect, b)modulatory effect on microcirculation, c)the effect on skin permeability allowing increased diffusion of drugs [1][2][3].

Zusammenfassung Einleitung Die oberflächliche Venenthrombose (OVT) der Beinvenen ist mit einer Inzidenz von 0,5–1/1000 Einwohner/Jahr ein häufiges und mit einer Frequenz assoziierter thromboembolischer Ereignisse in bis zu 25 % der Fälle ein ernst zu nehmendes Krankheitsbild. In der vorliegenden Übersichtsarbeit sollen alle relevanten Aspekte der OVT basierend auf der aktuellen wissenschaftlichen Literatur behandelt werden. Methoden Die systematische Übersicht umfasst das Wissen über die OVT basierend auf dem gesamten, systemisch recherchierten Erkenntnismaterial in den Datenbanken PubMed und Google Scholar. Eine Literaturrecherche in PubMed von Publikationen (1968–2018) mit den Suchbegriffen „Superficial vein thrombosis; thrombophlebitis“ ergab 102 aussagekräftige Arbeiten, die nach Vorlage der Originalarbeit ausgewertet wurden. Teil 2 der Arbeit behandelt Aspekte zur Therapie. Ergebnisse Die Bewertungskriterien der Therapie der OVT sind in den einzelnen Leitlinien und Übersichtsarbeiten z. T. unterschiedlich. Andererseits ist in den letzten Jahren eine zunehmende Stringenz in den Therapieempfehlungen festzustellen, die vom Ausprägungsgrad der OVT abhängig sind. Zu deren Prinzipien gehören die Mobilisation des Patienten und das Tragen eines medizinischen Kompressionsstrumpfes der Kompressionsklasse II. Die medikamentöse Therapie umfasst den Einsatz von Fondaparinux oder niedermolekularen Heparinen. Fondaparinux ist derzeit das einzig zugelassene Medikament für diese Indikation ab einer Thrombusausdehnung von mindestens 5 cm. Rivaroxaban in einer Tagesdosis von 10 mg war gegenüber Fondaparinux in einer randomisierten Studie nicht unterlegen; dies führte jedoch nicht zu einer Zulassung für die Indikation der OVT. Eine therapeutische Antikoagulation hingegen wird empfohlen, sobald sich das proximale Thrombusende 3 cm oder näher zum Übergang in das tiefe Venensystem befindet. Nichtsteroidale Antirheumatika reduzieren das Risiko einer OVT-Extension oder eines Rezidivs und wirken schmerzlindernd. Die Therapie mit Externa bietet nur lokale symptomlindernde Effekte. Eine operative Therapie der OVT kann in bestimmten Situationen erwogen werden, um Varizen, Schmerzen und das Risiko einer Thrombusaszension in einem Behandlungsschritt zu beseitigen. Schlussfolgerung Aufgrund der Häufigkeit und des Risikos potenziell lebensbedrohender Komplikationen sollte der OVT der Beinvenen eine größere Beachtung zukommen, insbesondere mit Blick auf eine auf den individuellen Befund ausgerichtete Stadien-adaptierte Therapie.
