Effects of somatostatin/octreotide treatment in neonates with congenital chylothorax

Abstract: The influence of somatostatin/octreotide treatment on outcomes of neonates with congenital chylothorax remains controversial. We retrospectively reviewed our experience with somatostatin/octreotide therapy in neonates with this very rare disease.Fourteen neonates with congenital chylothorax who were treated with somatostatin (3.5–7 μg/kg/h, before 2016) or octreotide (1–6 μg/kg/h, after January 2016), along with traditional management between 2013 and 2016, were retrospectively reviewed in this observational s… Show more

“…The neonates with successful resolution of chylous effusion required tube drainage for a median duration of 28 days (12–65 days). This is similar to other studies where the median duration of chest tube drainage was 12.5 days (2–51 days) [ 19 ].…”

“…In managing a child with chylothorax drainage of the effusion is essential 3 . Other modalities in the management are dietary modifications and the other medical therapies like somatostatin and octreotide to diminish chyle flow so that the thoracic duct could heal 5 . Early surgical intervention would be warranted if medical management fails or complications develop from the chylothorax 1 .…”

“…First line of treatment of chylothorax is the thoracostomy drainage and respiratory support, but there are other modalities like octreotide 3 . If drainage is required for several weeks, pleurodesis with OK-432 (Picibanil), erythromycin, or povidone-iodine may be helpful 5 . Surgical management includes ligation of the thoracic duct 6 .…”
