Aim To quantitatively assess corneal endothelial changes after phacoemulsification with the fluid-based system compared to conventional ultrasound technique. Methods This prospective, randomized clinical study included patients with cataract who were randomly assigned either to have phacoemulsification with the fluid-based system (30 eyes of 30 patientsFGroup 1) or with traditional ultrasound (30 eyes of 30 patientsFGroup 2). Patients who were available at each follow-up visit (25 eyes in both groups) were enrolled in the statistical analysis. Endothelial function was evaluated by measuring central corneal thickness, central endothelial cell density (ECD), mean cell size, and coefficient of variation in cell size preoperatively, 10 days, 1 and 3 months, and 1 year after surgery. Statistical analyses were performed using two-way repeated measure ANOVA. Results An acute, reversible increase of central corneal thickness (CCT) was found 10 days after surgery, which was similar in both groups (P ¼ 0.35). ECD decreased, whereas mean cell size increased significantly immediately after surgery. However, the impairments were finished after 1 month. The alterations were similar in both groups (ECD: P ¼ 0.99; mean cell size: P ¼ 0.85). The coefficient of variation in cell size remained stable after surgery (P ¼ 0.08), and significant difference was not found between groups (P ¼ 0.99). The endothelial cell loss (ECL) was 6.578.4% in Group 1 and 6.5711.7% in Group 2 (P ¼ 0.69). Conclusions Corneal endothelial changes were similar using the fluid-based system compared to the traditional ultrasound technique. The fluid-based method proved to be as safe as conventional ultrasound in cataract surgery.

ClinicalTrials.gov Identifier: NCT02089698 ABSTRACTPurpose: To compare the efficiency of surgical procedures using three phaco tip designs in torsional phacoemulsification using the bevel-down technique. Methods: In this prospective, comparative, masked study, patients were randomly assigned to have torsional coaxial microincision cataract surgery using the mini-flared 45-degree Kelman tip, reversed mini-flared 30-degree Kelman tip, or Sidewinder 30-degree Kelman tip. Clinical measurements included preoperative and 3-month postoperative corrected distance visual acuity (CDVA), endothelial cell counts (ECC), and preoperative and 1-day postoperative central corneal thickness (CCT). Intraoperative measurements included phaco time, torsional time, aspiration time, case time, cumulative dissipated energy (CDE), and balanced salt solution volume (BSS). Results: The study evaluated 150 eyes of 150 patients. Intraoperatively, there was no statistically significant difference in cumulative dissipated energy, case time, torsional time, and aspiration time between the three tip configurations. However, less phaco time was used with the mini-flared 45-degree Kelman tip (p=0.02) than that with the Sidewinder 30-degree Kelman tip or reversed mini-flared 30-degree Kelman tip. The mini-flared 45-degree Kelman tip and the reversed mini-flared 30-degree Kelman tip required significantly less balanced salt solution volume than that required by the Sidewinder 30-degree Kelman tip (p=0.009). There was no statistically significant difference in corrected distance visual acuity and endothelial cell counts between tips 3 months postoperatively (p>0.05). Conclusion: All three tips were effective with no intraoperative complications. When using torsional phacoemulsification through microincisions and the prefractu re technique with the bevel-down technique, the mini-flared 45-degree Kelman tip required a lower mean phaco time than the reversed mini-flared 30-degree Kelman tip and the Sidewinder 30-degree Kelman tip.

INTRODUÇÃOAs técnicas e resultados das cirurgias de catarata têm mudado muito nos últimos anos, especialmente após a introdução da facoemulsificação por Kelman (1) .A primeira publicação de remoção do cristalino in vivo realizada por Kelman em 1967 (1) inicia a aproximação às técnicas atuais de facoemulsificação. Desde então as inovações não cessaram. Novas técnicas de facoemulsificação, de fratura e conquista dos núcleos, novos materiais viscoelásti-cos e novas lentes intra-oculares (LIO), surgem a cada dia melhorando significativamente os resultados da cirurgia de catarata (2) .A indústria farmacêutica tem exercido grande participação no desenvolvimento de tecnologias. Os modernos aparelhos de facoemulsificação, perAvaliação do desempenho da tecnologia NeoSoniX TM em cirurgias de catarata Objetivos: As técnicas e resultados da cirurgia de catarata melhoraram drasticamente com o avanço tecnológico. Um dos motivos para melhoria seria a menor utilização de ultra-som com novos aparelhos de facoemulsificação. O objetivo deste estudo é confirmar na prática a vantagem teórica da caneta NeoSoniX TM . Métodos: Foi realizada uma análise na qual os autores compararam resultados dos parâmetros registrados pelo aparelho de facoemulsificação Legacy durante 300 cirurgias de catarata realizadas com caneta de ultra-som (handpiece) convencional e 100 com a caneta NeoSoniX TM . Todas as cirurgias foram realizadas pelo mesmo cirurgião, com a mesma técnica cirúrgica. A escolha da "handpiece" foi aleatória, sendo comparadas, em relação ao grau de dureza das cataratas, os parâmetros de tempo do ultra-som, quantidade de solução salina balanceada e porcentagem de "average power" utilizadas durante as cirurgias. Resultados: Foi realizada análise retrospectiva numa série cronológica de casos, mostrando que em qualquer que seja o grau de dureza da catarata, o tempo de utilização do ultra-som com a caneta NeoSoniX foi inferior ao da caneta convencional (p<0,000001). O volume de solução salina balanceada utilizado durante as cirurgias foi menor a favor da tecnologia NeoSoniX (p<0,000001). Em relação à porcentagem de potência do ultra-som utilizada com as diferentes tecnologias, encontrou-se valor inferior no grupo da caneta NeoSoniX (p<0,000001). Conclusões: Houve superioridade significativa no uso da caneta NeoSoniX comparada à caneta convencional do aparelho Legacy nas cirurgias de catarata, independente do seu grau. RESUMO
