Background. At present, dermal fillers based on hyaluronic acid (HA) represent the most popular intervention of dermoesthetic medicine for the treatment of skin aging. Recent studies have shown that the combination of HA chains of different lengths and molecular weights improves tissue repair and regeneration through a synergistic mechanism. Profhilo® is a product available that has been on the market since 2015 and is based on stable, hybrid, and cooperative complexes (HyCoCos) produced by means of NAHYCO® Hybrid Technology, which is an innovative thermal process that rules out the use of any chemical reagents. The result is a filler with high biocompatibility and low viscosity that favors optimal diffusion at the tissue level to obtain the target bioremodeling of the facial contour. The objective of this review is to provide data from the overall postmarketing experience after 3 years of use and more than 40,000 patients treated with the medical device. Methods. All spontaneous postmarketing adverse event (AE) reports received from physicians and healthcare professionals worldwide between February 9, 2015, and February 8, 2018, associated with the use of the studied medical device and sent to the IBSA global safety database were analyzed. Results. In total, 12 adverse event reports were logged in the global database, and none were considered serious. Early-onset injection site reactions, i.e., swelling, edema, redness, ecchymosis, and erythema, were the most frequently observed. Late-onset local reactions (e.g., swelling, nodules) followed. The genesis of these reactions was considered, both by the reporting physician and IBSA, as being local reactions of hypersensitivity and/or due to inappropriate injection techniques. In no case was the product held liable for direct damage. All events resolved without any complications according to the treatment guidelines. Two late-onset reactions were collected. Conclusions. Although underreporting of minor events cannot be ruled out, the overall number of reports is very low, thereby supporting the high tolerability and safety of the product. After 3 years of postmarketing experience, the safety profile of the studied medical device is favorable and consistent with the product information.

Background: Beauty standards vary by race and region, which must be accounted for in creating beauty. Chinese have unique features, including a depressed nose, flat midface, and small chin, and have different cosmetic concerns. We performed a consensus study on filler injections based on the Chinese standard of beauty due to a lack of such study. Methods: We organized the YVOIRE Consensus Group, including 5 plastic surgeons. We discussed common problematic areas seen in Chinese and described techniques based on our experience with hyaluronic acid fillers, while considering peer-reviewed articles, followed by multiple consensus-developing sessions. Results: Chinese faces are characterized by retruded forehead, chin, and low nasal bridge, with certain features believed to bring good fortune. Therefore, profiles of the forehead, nose, lip, and chin have substantial effects on attractiveness. The demand for cosmetic procedures is high among the young generation who particularly desire nose and chin augmentation. Attractive Chinese facial shapes are characterized by a long, slender facial shape and pointed chin. Lips are narrow and thick. When injecting fillers for Chinese, it is necessary to apply special methods that produce results consistent with these characteristics. Conclusions: Understanding the concept and demand of beauty depending on race and region is important. Patients should be evaluated before aesthetic procedures. Maintaining hygiene during the procedure is crucial. Moreover, methods for preventing pain are essential. Fillers should be injected into the correct anatomical site and layer to minimize side effects and maximize effectiveness.

O ácido hialurônico (AH) é um dos preenchedores dérmicos temporários mais usados na correção de rítides, linhas e sulcos faciais por ser seguro e eficaz. Este ácido é natural no organismo humano e dentre as suas funções destaca-se a manutenção do volume, sustentação, hidratação e elasticidade da pele. Múltiplos preenchedores dérmicos estão disponíveis variando em relação às características físicas e químicas, o que influi em seus efeitos. O conhecimento do profissional sobre a anatomofisiologia da pele, assim como as características e propriedades reológicas dos AH, bem como a escolha adequada do produto para a região em que será aplicado e a experiência na aplicação são de suma importância para a obtenção de um resultado natural e duradouro. Os objetivos deste estudo foram apresentar as principais vantagens do uso do AH como preenchedor dérmico, suas indicações, contraindicações, reações adversas e possíveis complicações no tratamento para o rejuvenescimento facial humano. Trata-se de um estudo de revisão bibliográfica narrativa. Foram consultadas as bases de dados virtuais PubMed/MEDLINE e Portal Periódicos Capes. Estudos demonstraram que a aplicação inadequada do AH em planos teciduais incorretos é crítico na indução de reações adversas, devido à distensão dos tecidos e ao trauma. Observou-se que reação inflamatória transitória, leve ou moderada e equimoses podem ocorrer alguns dias após o procedimento. Estudos revelam que bons resultados do preenchimento dérmico facial se mantêm por um período variável de 6 a 18 meses. Os procedimentos são rápidos, ambulatoriais, sem necessidades de testes ou afastamentos de atividades diárias, com reações adversas mínimas e complicações raras. Avaliação cuidadosa do paciente, planejamento terapêutico adequado e técnica apurada são fundamentais para alcançar os melhores resultados com o tratamento.
