Effectiveness and failure factors of manual aspiration using a small needle for large pneumothorax in stable patients

Abstract: Background: Manual aspiration as the initial management of a large pneumothorax in a clinically stable patient has been reported to be safe and effective. However, the effect with smaller needles, the number of aspiration, the indication other than spontaneous pneumothorax and failure factors are unknown. We assessed the effectiveness and failure risk factors of manual aspiration up to three using a 20-or 22-gauge (G) needle in patients with a large, clinically stable pneumothorax. Methods: We included 107 clin… Show more

“…However, it may be less effective in a large area of pneumothorax and has a high risk of recurrence in a short period of time. 14 In a study conducted by Homma, et al (2022), needle aspiration can be the treatment of choice in clinically stable patients three times, regardless of the size of the pneumothorax. 5 In this case, we obtained pneumothorax with an interpleural distance of more than 2 cm, which was accompanied by shortness of breath.…”

“…Aspiration failure has been reported more frequently in patients with a traumatic PTX with an inter-pleural distance > 20 mm at the level of the hilum. Additionally, if the pneumothorax is large or under tension, needle aspiration alone may not be sufficient to achieve adequate lung re-expansion [27].…”

“…Doctrinaire chest tube removal criteria may already be outdated for modern medicine ( 1 , 12 - 19 ). Postoperative pain was not assessed in this study, but chest tube and pain are highly correlated ( 18 , 19 , 24 ). Early chest tube removal may affect postoperative complications and shorter hospitalization.…”
