АННОТАЦИЯ С целью изучения экспрессии фактора роста эндотелия сосудов (VEGF), костного морфогенетического протеина-15 (BMP-15) и коннексина-37 (Cx-37) в яичниках крыс с моделью функциональных кист яичников и при их коррекции был проведен эксперимент на половозрелых белых крысах-самках линии Wistar. Группа I (основная, n = 25)-моделирование функциональных кист яичников (введение фолликулостимулирующего гормона) с последующим лечением бевацизумабом. Группа II (сравнения, n = 35)-моделирование функциональных кист яичников без последующего лечения. Группа контроля (n = 20)-интактные животные. Крысы выводились из эксперимента на 7, 15, 30-е сутки. Проведено иммуногистохимическое исследование экспрессии VEGF, BMP-15, Cx-37. Моделирование функциональных кист яичников у белых крыс сопровождалось увеличением удельного объема сосудов в яичниках, усилением экспрессии VEGF в стенке кисты, избыточной стимуляцией фолликулогенеза, сопровождавшейся усилением экспрессии BMP-15 и Cx-37 на ранних сроках эксперимента. Введение ингибитора сосудисто-эндотелиального фактора роста вызывало уменьшение количества и удельного объема кист, снижение экспрессии VEGF и приближение удельного объема сосудов и экспрессии VEGF к значениям контрольной группы на 15-е сутки. К 30-м суткам экспрессия BMP-15 и Cx-37 не отличалась от показателей группы интактного контроля. Таким образом, введение бевацизумаба ускоряет регресс функциональных кист яичников за счет снижения васкуляризации в стенке кисты, сопровождающейся снижением экспрессии VEGF. Выявленное уменьшение количества растущих фолликулов, усиление процессов атрезии в фолликулах и изменение экспрессии факторов овосоматического взаимодействия (BMP-15, Cx-37) имеют кратковременный характер. Ключевые слова: бевацизумаб, лечение функциональных кист яичников, сосудисто-эндотелиальный фактор роста, костный морфогенетический протеин-15 (BMP-15), коннексин-37 (Cx-37).

Introduction: Endometriosis affects up to 1 every 5 women at their reproductive age, with variable and complex symptomatology. Patients may be asymptomatic but may have pain episodes or subfertility. Its negative impact is on patients' health and quality of life. Objective: it was to investigate the serum and peritoneal fluid (PF) concentrations of Angiopoietin-2, Interleukin-1β, and Vascular Endothelial Growth Factor, aiming to evaluate their diagnostic performance in endometriosis. Methods: Serum and peritoneal fluid samples were taken from 112 women undergoing laparoscopy for infertility, pelvic pain or adnexal masses. 61 diagnosed with endometriosis and 51 controlled. Primary outcome was to estimate serum and PF concentrations of Angio-2, IL-1β and VEGF and secondarily correlate these concentrations to disease stages thus assuming their diagnostic potential. Results: Significant differences were found between patients and control as regards serum and PF concentration of all studied markers except serum IL-1β. Serum Angio-2 and PF VEGF showed a significantly higher level in more advanced stages of endometriosis. PF VEGF showed a positively significant correlation with the stage of the disease, spearman coefficient t = 0.442 p = 0.014. PF concentrations of Angio-2 and Serum VEGF did not show significant pattern changes with stage-related levels. Diagnostic potential of serum and PF concentrations of the 3 markers were assessed by the ROC curve. Angio-2 proved an excellent diagnostic ability for endometriosis. PF and serum VEGF proved an equal diagnostic performance, whereas, PF IL-1β was the least efficient. Based on the results, we suggested preliminary serum threshold values for these markers to be used as diagnostic or follow-up landmarks with relatively acceptable sensitivity, specificity, positive and negative predictive values. Conclusion: Non-invasive predictive biomarkers for endometriosis were Serum Angio-2, IL-1β, and VEGF independently or in combination with the estimated threshold values. Serum Angio-2 merit is considered as a novel marker for endometriosis due to its diagnostic power.

Characterized by its cyclical regeneration and differentiation, the endometrium is one of the most dynamic tissues of the human body. As a main player during implantation and later development of the embryo it has a unique and extremely important role in the survival of species. This study is a review of the current literature focused on the cyclical restructuring of the endometrium and the morphological and cellular alterations during the different phases of the reproductive cycle. These changes confer specific receptive capabilities for implantation to take place. The mechanism of implantation is addressed as well as possible receptivity obstacles that can influence this process. More specifically, we discuss the involvement of immune cells in the establishment of implantation and its consequences for a successful pregnancy. A deep knowledge of the mechanisms involved in the regulation and transformation of the endometrium and embryo implantation is essential to understand disorders that can influence fertility and women health.
