Effect of uric acid lowering therapy on the prevention of acute kidney injury in cardiovascular surgery

Abstract: In this study, rasburicase showed no benefit on postoperative serum creatinine in hyperuricemic subjects undergoing cardiac surgery. However, the observation that markers of structural renal injury such as uNGAL tended to be lower in rasburicase-treated subjects suggests potential different effects of uricase treatment on hemodynamic alterations in renal function versus structural mechanisms of kidney injury.

“…Gaipov A et al also reported higher prediction capability of AKI at 24 and 48 hrs with UA and with NGAL, in open heart surgery patients 9 . Apart from an early marker of AKI, UA is now also becoming a potential therapeutic target in kidney diseases 10,11 . Serum IL-6 has been reported has a early marker of AKI in few studies 12,13 .…”

“…Very elevated serum uric acid levels contributing to acute renal failure has also been described in young infants after cardiovascular surgery [4], and has also been proposed as a risk factor for acute kidney injury in adult patients undergoing high risk cardiac surgery [5,6]. A rise in serum uric acid is also seen in toxic and ischemic acute kidney injury where it is thought to be the result of both increased generation and decreased excretion of uric acid [7].…”
