Effect of Nd:YAG laser capsulotomy on the risk for retinal detachment after cataract surgery: systematic review and meta-analysis

Abstract: Nd:YAG laser capsulotomy increased the risk for retinal detachment. The associations were stronger for Asian than non-Asian populations and for extracapsular cataract extraction than for phacoemulsification.

“…The role of YAG laser capsulotomy as a cause of RD remains controversial, although recent studies consider it to be a risk factor [10]. YAG capsulotomy is a common procedure, despite being a minimally invasive procedure, several complications have been reported in the posterior segment, such as the rupture of the anterior hyaloid face, macular oedema, retinal tears and RD [25][26][27], secondary a a breakdown of the physical barrier, biochemical alterations and acoustic shock waves in the vitreous body, which can cause changes similar to those that take place when posterior capsule rupture occurs during cataract surgery [10,28]. Our study didn´t nd statistically signi cant associations with YAG capsulotomy, probably due to the small number of patients who underwent this procedure in our sample.…”

“…The risk factors for PRD are known to include: age, being more frequent in patients under 60 years of age [4] given that most of these lack the "protective factor" of having posterior vitreous detachment at the time of surgery [5]; gender, being more frequent in men [6] , ; myopia [2,7]; intra-operative complications, such as rupture of the posterior capsule during phacoemulsi cation [8]; the presence of lattice degeneration [9]; and having been subjected to YAG laser capsulotomy, although this remains somewhat controversial [10,11] , . Given that cataract surgery is currently the most common ophthalmic surgical procedure performed in our clinical setting [12], and that there is a boom of clear lens extraction in refractive surgery at younger ages, the incidence of PRD is expected to increase in the coming years.…”

“…Nevertheless, the procedure itself is not devoid of complications, including damage to the IOL, such as IOL pitting or dislocation, cystoid macular oedema, and retinal detachment. 3,[11][12][13] In addition to the potential complications, PCO also places a financial burden upon the healthcare system. 14 Consequently, there is significant interest in PCO prevention.…”

“…Neodymium:YAG (Nd:YAG) laser capsulotomy is the most common and efficient approach to manage PCO. Nevertheless, the procedure itself is not devoid of complications, including damage to the IOL, such as IOL pitting or dislocation, cystoid macular oedema, and retinal detachment 3,11–13 . In addition to the potential complications, PCO also places a financial burden upon the healthcare system 14 .…”

“…6 In addition, RLE patients can be more likely to undergo YAG capsulotomy, adding on further risk of complications. 3 Thus, it is reasonable to be cautious toward early RLE while additional evidence continues to emerge.…”
