Effect of gender on strength gains after isometric exercise coupled with electromyographic biofeedback in knee osteoarthritis: A preliminary study

Abstract: The results suggest that gender did not affect gains in muscle strength by isometric exercise coupled with electromyographic biofeedback in patients with knee OA.

“…The subjects laid down on the floor by straightening their knees, propping a rolled hand towel behind the knee, and bending the knee joint naturally. They were then instructed to straighten the knee (“extend the knee and pull the patella towards themselves”) maximally for 5 sec beyond the threshold level to monitor the VMO activity 31 ) . During the actual training session, the subjects were allowed to relax for 10 sec after at least 5 sec of contracting the VMO beyond the threshold level.…”

“…EMG-BF is a physiotherapeutic technique that is often used with other exercise programs such as strength training to reduce pain and improve muscle strength in people with PFPS and knee osteoarthritis [ 26 , 35 , 36 , 37 , 38 , 39 ]. EMG-BF-guided strength training encourages active participation, which motivates patients to play an active role during the training period in achieving their desired therapeutic rehabilitation goals [ 37 ]. EMG-BF-guided strength training is conducted by a special type of biofeedback device that records muscle tone and tension measured in microvolts via the application of surface or needle electrodes [ 40 , 41 ].…”

“…It was applied distal to the patella to unload the infrapatellar fat pad (as illustrated in Figure 3 ). First, hypoallergenic underwrap tape was applied to prevent skin irritation; then, rigid (McConnell) tape was applied to maintain the patella by medially pulling the skin and patella [ 37 ]. Tape was removed after the outcome measure or if the participant experienced itching, redness, or discomfort.…”

“…The voluntary activity threshold of the patient was set at 20% less than the calculated mean. 28 Each time the patients in the EMGBF group managed to contract their muscles beyond the established threshold, the device gave them positive feedback, which could be a puzzle getting completed one piece at a time or an animated car moving across the monitor. The physician increases the threshold at each session, according to the patients’ strength, so that they become encouraged to increase their muscle activity.…”
