OBJECTIVE: This study aimed to assess the amount of moisture present in the TWS of dental units in dental teaching hospitals (DTH) and private clinics (PC) across Karachi. METHODOLOGY: A total of 285 dental units were included in the study out of which 250 belonged to DTH and 35 to PC. Gushing was initially performed on hand to remove visible moisture from the TWS. The number of gushes required to remove visible moisture were recorded and the moisture-sensing device was used to assess the amount of invisible moisture in dry-air released from the TWS. SPSS v 22 was used to compare data of DTH and PC with the help of Mann Whitney U test. The pre- and post-exposure humidity of the sensing chamber was analyzed through Wilcoxon signed rank test. p value of < 0.05 was considered as significant. RESULTS: The power of the study was found to be > 99%. The moisture was present in 77.6% of the TWS in DTH and 37.1% in PC (p <0.001). Significantly higher number of hand gushes were required in DTH to eliminate the visible moisture as compared to PC (p= 0.022). Similarly, TWS in DTH were seen to liberate significantly increased amount of invisible moisture as compared to PC (p-value<0.001). CONCLUSION: Alarmingly high number of three-way syringes of dental units in dental teaching hospitals had moisture. This moisture can jeopardize restorative treatment and may expose patient to lethal microbes. KEYWORDS: Three way syringe; Restorative; Moisture; Resin Composite; Contamination

Application of intracanal medicaments may affect the physical properties of root dentine. Calcium hydroxide (CH), a gold standard intracanal medicament, has proven to decrease root dentine microhardness. A natural extract, propolis, has been shown to be superior to CH in eradicating endodontic microbes, but its effect on the microhardness of root dentine is still not known. This investigation aims to evaluate the effect of propolis on root dentine microhardness compared to calcium hydroxide. Ninety root discs were randomly divided into three groups and treated with CH, propolis, and a control. A Vickers hardness indentation machine with a load of 200 g and dwell time of 15 s at 24 h, 3, and 7 days was used for microhardness testing. ANOVA and Tukey’s post hoc test were used for statistical analysis. A progressive decrease in microhardness values was observed in CH (p < 0.01), whereas a progressive increase was observed in the propolis group (p < 0.01). At 7 days, propolis demonstrated the highest microhardness value (64.43 ± 1.69), whereas CH demonstrated the lowest value (48.46 ± 1.60). The root dentine microhardness increased over time when propolis was applied, while it decreased over time after application of CH on root dentine sections.

Purpose. Postendodontic pain is one of the problems of root canal therapy. This clinical study aimed to evaluate the effect of infiltration injection of dexamethasone and methylprednisolone on postendodontic pain in patients with necrotic pulp. Methods. A total of 80 volunteers with necrotic pulp teeth were included and assigned to two groups (n = 40). After the administration of local anesthesia and before root canal therapy, in group 1, an infiltration injection of 1 ml of dexamethasone was done and in group 2, an infiltration injection of 1 ml of methylprednisolone was done in the buccal vestibule of each tooth. Patients’ pain was reported using a visual analogue scale at pretreatment and 6, 12, 24, and 48 hours after treatment. Results. There was no significant difference between the two groups receiving dexamethasone and methylprednisolone at pretreatment and 6, 12, 24, and 48 hours after endodontic treatment. Conclusions. Infiltration injection of dexamethasone and methylprednisolone had a significant effect in reducing pain after the endodontic treatment in necrotic pulp teeth, but between 6 and 12 hours, methylprednisolone had significantly more effect on pain relief than dexamethasone. Overall, the use of any of these drugs to reduce postendodontic pain is recommended.
