Effect of Decentration on the Optical Performance in Multifocal Intraocular Lenses

Abstract: Aims: To evaluate the influence of decentration on optical performance in multifocal intraocular lenses (IOLs) using eye models. Methods: This study evaluated 4 types of multifocal IOLs (ReSTOR SA60D3, Alcon; TECNIS Multifocal ZM900, AMO; ReZoom, AMO; SFX-MV1, Hoya). The evaluations were based on measurements of the near and far modulation transfer function (MTF) and visualized actual near images (newspaper) using eye models with the IOL horizontally displaced 0, 0.25, 0.5, 0.75 and 1.0 mm from the center. “…19,20 Capsule contraction has been shown to cause changes in IOL position in the late postoperative period, 21 although the effect of this seems to vary depending on the haptic design of the IOL. 19 Considering that small amounts of decentration or tilting of a diffractive multifocal IOL can generate deterioration in visual performance, 22 late positional IOL changes could contribute to late visual and aberrometric changes. Future studies should confirm this using imaging techniques to evaluate the anatomic position of the IOL in the capsular bag after its implantation.…”

“…However, a large angle kappa and MIOL decentration from the visual axis should also be considered. Soda & Yaguchi (2012) evaluated the influence of horizontal decentration on optical performance in different MIOLs using an eye model. He found that clinically relevant effects are not to be expected up to a decentration of 0.75 mm.…”

“…Although some authors have reported good clinical results with multifocal IOLs,14 15 many practitioners have still some concerns about the performance of this IOL modality due to the significant rates of postoperative halos and glare perception associated with some of these implants16 that may affect daily tasks and quality of life 17. Likewise, visual alterations induced by non-centred IOLs18 and the low tolerance to mild residual refractive errors that may yield to a significant reduction of UDVA19 are other concerns related to multifocal IOLs.…”
