In recent years, calcium phosphate cements (CPCs) have frequently been used as bone substitutes in the field of orthopedic surgery. When CPC is used as a bone substitute in vivo, blood contamination is unavoidable. To date, however, no detailed study has been conducted focusing on how the physical properties of CPCs would change under the influence of blood. In this study, the effects of blood contamination on Biopex-R (BPR, PENTAX, Tokyo) are examined in vitro and in vivo. The compressive strength of BPR after setting decreased depending on the amount of contaminating blood. The BPR, which has set in vivo, not only has a fragile surface due to the contamination by blood, but also has a propensity to shorten and be destroyed during the early postoperative stage, especially in the bone exposed to loads. On the other hand, radiographic and histological features in vivo indicated that the absorption and the bone replacement of BPR were stimulated by blood contamination. In the clinical evaluation, the patient's own peripheral venous blood was added to the BPR. One year after the surgery, the absorption was noted around the hardened BPR. To advance CPCs (including BPR) as bioabsorbable bone replaceable materials, it is essential to utilize the patient's own blood in combination with the CPC.

The spherical β-calcium orthophosphate (β-Ca 3 (PO 4) 2 : β-TCP) agglomerates have been prepared by a spray-pyrolysis technique. The hollow spherical agglomerates were obtained by heating the spray-pyrolyzed powder at 900 for 10 min. The cylindrical specimen was fabricated mixing the calcium-phosphate paste (CPP) with 10-50 mass% β-TCP agglomerates, using malaxation liquid. The setting time of CPP specimen increased from 8.5 to 17 min with increasing amount of β-TCP from 10 to 50 mass%. The CPP specimens with β-TCP addition were immersed into the simulated body fluid (SBF) at 37±0.2 for various times. The compressive strengths of CPP specimens with β-TCP addition reached maxima, e.g., 42.8 MPa for 10 mass% β-TCP addition and 7.5 MPa for 50 mass% β-TCP addition, respectively, after the immersion of these specimens in SBF for 3-7 d. Crystalline phases of the CPP specimens after the immersion in SBF for 7 d were HAp and β-TCP. The total porosity of CPP specimen increased with increasing amount of β-TCP and attained 67.1% for 50 mass% β-TCP addition.

Hydroxyapatite (Ca 10 (PO 4 ) 6 (OH) 2 ; HAp) is one of inorganic components of bone and teeth, and has an osteoconductivity and execellent biocompatibility. Thus, the HAp has been used as biomaterials for bone graftings. Clinically-used HAp cements are set on the basis of the acid-base reaction of Ca 4 O(PO 4 ) 2 and CaHPO 4 . This mechanism accompany the changes of pH during hardening, leading to inflammation around tissues, together with slow setting time. We have succeeded to develop novel HAp cement, which is created by mixing the HAp powder modified with inositol phosphate (IP6) and water. The present IP6-HAp cement is set based on the chelate-bonding of IP6 without the acid-base reaction. The IP6 is not only biocompatible but also has able to chelate to some metal ions as strongly as EDTA. In the present study, we fabricated four kinds of IP6-HAp cements using commercially-available HAp powders with diferent morphology and specific surface area (SSA): HAp-100 (fine particle), HAp-200 (hexagonal shape), HAp-400 (plate shape), and s-HAp (spherical shape). Among the cement specimens examined, the IP6/HAp-100 cement derived from HAp-100 powder had maximum compressive strength of 10 MPa under following condition: Powder/Liquid (P/L) ratio＝1/0.45[w/w]. The HAp-100 powder was composed of microcrystals with the highest SSA of 62.4 m 2 ･g -1 among the HAp powder used in the present work. On the other hand, the compressive strength of the IP6/HAp-200 cements showed the lowest value of 2 MPa under following condition: P/L ratio＝1/0.35[w/w]. The HAp-200 powder of a starting material was composed of hexagonal-shaped crystals, and had the lowest SSA of 7.0 m 2 ･g -1 . These results indicate that the compressive strength of novel IP6/HAp cements was affected by powder properties, especially, the morphology and SSA of starting HAp powders.
