Comparison of hemostatic efficacy of topical Ankaferd Blood Stopper on heparinized and nonheparinized rats in bleeding related to liver injury

Purpose: In this study, hemostatic efficacy of Ankaferd Blood Stopper (ABS), a new generation hemostatic agent, was compared in the presence of heparin effect. Methods: Forty-eight Wistar albino rats were divided into two main groups as heparinized and nonheparinized, and these two main groupswere divided into six subgroups as control, Surgicel and ABS (n = 8). Grade 2 liver injury was performed on rats as standard. All groups were compared in terms of weight, laceration surface area, prothrombin time (PT), activated partial thromboplastin time (aPTT), international normalized ratio (INR), bleeding time, bleeding amount, hemoglobin (Hb) levels, macroscopic and microscopic reactions to the agent used. Results: Whereas there was no statistically significant difference between weight, laceration surface area, PT, INR and preoperative Hb values in the heparinized and nonheparinized groups, postoperative Hb, bleeding time, bleeding amount and aPTT values were statistically different (p < 0.05). In the heparin-hemostat interaction, the ABS group had the lowest bleeding in the heparinized group in terms of the amount of bleeding compared to the control and Surgicel groups (F = 0.764; p = 0.047). In macroscopic and microscopic comparison, there was no difference between the groups in terms of cell necrosis andfresh bleeding (p > 0.05), it was found that the Surgicel group had statistical significantly higher reaction scores (p < 0.05) than the other groups in terms of other parameters. Conclusions: Ankaferd Blood Stopper can be safely and effectively used in surgical practice and in patients with additional diseases requiring heparinization, since it causes minimal reaction in the liver and decreases the amount of bleeding especially in the heparinized group.

IntroductionDiabetes mellitus (DM) is a chronic, life-long metabolic disease occurring worldwide. Affected individuals require continuous follow-up and suffer numerous disease-related complications. The high levels of blood glucose that characterise DM contribute to an increase in free radical production, enhanced oxidative stress, and changes in antioxidant capacity (1-3).Streptozotocin (STZ) is the chemical agent most frequently used to induce experimental diabetes in animal models. By binding to glucose receptors in the plasma membrane, STZ blocks glucose-induced insulin secretion (2,3) and alters blood insulin and glucose concentrations.Among the many effects of free radical production is lipid peroxidation (LPO). Oxidation of membrane polyunsaturated fatty acids disrupts cell structure and function and induces cytotoxic, hepatotoxic, mutagenic, and genotoxic effects related to the release of aldehydes (4,5).The tripeptide glutathione (GSH) is found in the cells of all organisms. In humans, GSH concentrations are highest in the liver; in hepatocytes, GSH accounts for 90% of nonprotein sulfhydryl groups. As an important reducing agent and antioxidant, GSH maintains the cellular oxidoreduction balance and protects cells against the toxic effects of oxidants, whether of endogenous or exogenous origin (5-7).Ankaferd BloodStopper (ABS) (Ankaferd Health Products Ltd., İstanbul, Turkey) is a medicinal extract from the plants Thymus vulgaris, Glycyrrhiza glabra, Vitis vinifera, Alpinia officinarum, and Urtica dioica (7,8). ABS is not only an effective haemostatic agent but also Background/aim: Diabetes mellitus inhibits wound-induced angiogenesis, impairs the wound healing process, and leads to the development of chronic wounds. Ankaferd BloodStopper (ABS) is a new and promising local haemostatic agent. Although the mechanism of ABS-mediated haemostasis is well established, little is known about the associated histological and biochemical tissue reactions. The aim of this study was to evaluate the effects of this new-generation local haemostatic agent on short-term soft-tissue healing in streptozotocin (STZ)-treated rats. Materials and methods:The 24 Wistar albino rats used in this study were divided into STZ-treated (STZ, n = 12) and nontreated groups (control, n = 12). Four days prior to surgery, rats in the STZ group were subcutaneously administered 60 mg/kg STZ intraperitoneally, while rats in the control group were administered 1 mL saline/kg. An incision was made in the dorsal dermal tissue of all rats, and either ABS or no haemostatic agent (NHAA) was applied to the wound before suturing. All of the rats were euthanised on postoperative day 4. Blood and skin samples were evaluated biochemically and histologically. Results:The results showed that STZ treatment impaired soft-tissue healing, assessed by measuring glutathione and lipid peroxidation levels. Several alternative ways for dealing with rodents or correcting mouse damage have been passed down through the generations. Anticoagulant rodenticides were developed in the late 1940s, and they were a significant step forward in rodent management. In many locations, warfarin and its sodium counterpart are used to control rodents. Warfarin is a human drug that is used to treat blood clotting problems. These anticoagulants are administered as rodenticides in rats and mice, causing hemorrhaging and ultimately death. In this study, the acute oral warfarin of wild rats was determined. The effects of different doses of warfarin (¼ 27.5, 9 mg/kg.b.w and ½ 55, 18 mg/kg.b.w) on rat kidneys of males and females respectively, were examined. These effects included considerable elevation of kidney functions, remarkable raise in LPO level with a significant decrease in GSH, SOD, CAT, and NO activities. Concerning histopathological changes, there were changes in glomeruli and renal tubules represented by expanding in the lining epithelium of the renal tubules with vascular degeneration of the epithelial cells covering the tubules, throughout the inner cavities of tubules there were wide areas of necrosis and appearance of inflammatory cellular infiltration, lymphocytes, and monocytes in interstitial tissues. It was concluded that warfarin oral intake of wild rats produced clear dose-related renal toxicity with different pathological effects.
