BackgroundConstipation is a common, uncomfortable, and frustrating complication in patients with advanced cancer which in turn affects the patient’s quality of life. The aim of this study is to investigate the effects of laser acupuncture on constipation.MethodsThis is a prospective, single-center, double-blinded, randomized controlled trial study. A sample of 74 participants will be randomly assigned to either the verum or the sham laser acupuncture group in a 1:1 ratio. The participants will receive laser acupuncture or sham laser acupuncture once a day for consecutive six days on eight acupoints (LI4, LI11, PC6, ST36, ST37, ST39, SP4, and SP9). The number of complete spontaneous bowel will be used primarily for the evaluation, and symptoms of constipation, comfort levels during defecation, and colonic motility will be also be measured. Data will be analyzed using the chi-square test or t-test. Analysis of covariance will be used to examine the differences on symptoms of constipation between two groups and adjusted for their respective baseline levels. DiscussionThis trial may provide evidence for improving the short-term benefit for advanced cancer patients by relieving the symptoms caused by constipation. As a result, further reduce the usage of medications, and lower the side effects due to medications.Trial registration: ClinicalTrials.gov, NCT04318808. Registered on 24 March 2020.

Konstipasyon kanser hastalarında sık görülen semptomlardan biri olup, bu semptomun görülmesinde hareket azlığı, yetersiz sıvı alımı, kemoterapi, radyoterapi gibi tedavi yöntemleri ve opioid kullanımı gibi birçok faktör etkili olabilmektedir. Kanser hastalarının konstipasyon gelişme riski açısından yakından izlenmesi bu semptomun erken dönemde tanılanmasına katkı sağlar. Amaç: Bu çalışmanın amacı onkoloji kliniğinde yatan kanser hastalarında konstipasyon sıklığı/gelişme riski ve konstipasyon gelişmesini etkileyen bazı faktörlerin belirlenmesidir. Yöntem: Tanımlayıcı nitelikteki bu çalışma Ekim 2018-Şubat 2019 tarihleri arasında 18 yaşından büyük, kanser tanısı almış 129 hasta ile yürütülmüştür. Veriler 'Birey Tanılama Formu' ve 'Konstipasyon Risk Değerlendirme Ölçeği' kullanılarak toplanmıştır. Verilerin değerlendirilmesi Ki-kare, Mann-Whitney U testi analizi ile yapılmıştır. Bulgular: Hastaların yaş ortalaması 59 ± 1.12 yıl, %34.2 'si kadın, %65.8 'i erkek ve konstipasyon görülme sıklığı %62'dir. Hastaların konstipe olmasında sürekli opioid tedavisi alma, hastaneye yatış nedeni ve metastaz varlığına göre istatistiksel olarak anlamlı bir farklılık olduğu belirlenmiştir. Sürekli opioid tedavisi alan ve destek tedavi almak amaçlı hastaneye yatan hastalarda konstipasyon oranı daha yüksekti (p < .05). Konstipe olmayan hastaların ise konstipasyon gelişme riskinin orta düzeyde (13.1 ± 4.3) olduğu saptanmıştır. Sonuç: Çalışmamızda onkoloji kliniğinde yatan hastaların çoğunluğunun konstipasyon yaşadığı, sürekli opioid tedavisi alma, destek tedavi alma ve metastaz varlığının hastaların konstipasyon yaşamasında etkili olduğu ve konstipe olmayan kanser hastalarının ise orta düzeyde risk altında olduğu belirlenmiştir.

Background: Functional constipation is a disease with a high incidence, which has a bad effect on general health, mental health, and social functioning. However, current treatment is sometimes unsatisfactory. Acupuncture has been proven effective in some randomized controlled trials. Acupressure is a subtype of acupuncture and can be manipulated by the patients at home. But the evidence is limited now. This study aims to provide some strict evidence for the use of self-administered acupressure in the treatment of functional constipation. Methods: This 2-armed, parallel, nonspecific controlled, randomized trial will be conducted at The Third Affiliated Hospital of Zhejiang Chinese Medical University in Hangzhou. A total of 154 FC patients will be enrolled into the acupoint group and the sham acupoint group with a ratio of 1:1 into this trial and it will consist of a 2-week run-in period, an 8-week intervention period, and an 8-week follow-up period. The treatment will be done by the patients themselves at home twice a day and they should sign in on the WeChat APP every day to make sure they have done the acupressure. The outcome will also be collected in WeChat APP through the diary and questionnaires. For the one who is unable to use the WeChat, the print edition of the diary and questionnaires are provided and the supervision will be done by the short message. The primary outcome will be the proportion of participants whose CSBM≥3 during week 3 to 10. The secondary outcome will be the proportion of participants whose CSBM ≥3 between 2 groups in week 11 to 18, Spontaneous bowel movements, Bristol Stool Form Scale, Straining severity scores, Patient assessment of constipation quality of life, and Medicine use. Discussion: Acupressure is not an invasive method and can be done by the patient itself at home. We hope this trial will provide credible evidence to the application of self-acupressure for the management of severe chronic functional constipation. Trial registration: This trial has been registered at the Chinese Clinical Trial Registry (ChiCTR2000038594).
