Efavirenz decreases etonogestrel exposure

Abstract: Objectives The primary objective of this study was to characterize the pharmacokinetics of etonogestrel (ENG) released from a contraceptive implant in Ugandan women living with HIV who were receiving efavirenz (EFV)- or nevirapine (NVP)-based antiretroviral therapy (ART), compared to ART-naïve women over 24 weeks. Design Non-randomized, parallel-group study with 3 arms: ART-naïve, NVP-, or EFV-based ART (N=20 per group). Methods Sparse pharmacokinetic sampling of ENG, NVP or EFV were performed at screening… Show more

“…Aside from some reduction in effectiveness due to drug interactions with some drugs, the implant is as much as 180 times more effective than short-acting methods. [18] Even though efavirenz use reduces the effectiveness of the implant, by lowering levels of the active drug in Implanon (etonogestrel), [19,20] the implant still appears more effective in these patients than other contraceptive methods. Relative effectiveness -as well as the likelihood of side-effects -can be hard to convey in counselling.…”

“…[73] There are substantial drug interactions between efavirenz and Implanon, considerably reducing drug levels of etonogestrel, the active drug in Implanon. [74][75][76] Interactions between efavirenz and Jadelle appear even more pronounced than with Implanon. [77,78] Women using an implant, who are also taking efavirenz, have higher pregnancy rates than those taking other antiretrovirals.…”

“…Four PK studies now demonstrate markedly reduced etonogestrel or levonorgestrel concentrations in women concomitantly using efavirenz-containing ART [4447], with one study documenting three pregnancies among the 20 women using efavirenz-containing ART [45]. The same study also demonstrated statistically nonsignificant higher exposure to levonorgestrel among women concomitantly using nevirapine-containing ART as compared to women not on any ART [45].…”
