Ectopic Functioning Thyroid Tissue in the Thyroglossal Duct Detected by Radionuclide Imaging

Abstract: A 49-year-old man with a clinical diagnosis of Graves' disease was referred for a thyroid scan and radioactive ablation of the gland. Tc-99m pertechnetate scan revealed a diffuse toxic goiter and radiotracer concentration in the thyroglossal duct. He was given 10 mCi of I-131 orally as treatment of thyrotoxicosis. Repeat scanning of the anterior neck 2 days later revealed concentration of radioiodine in the diffusely enlarged gland and the thyroglossal duct as found on the Tc-99m pertechnetate scan, thus confi… Show more

“…On histological examination, ectopic thyroid may be found within the wall of the cyst with an overall frequency of !5%. It produces thyroid hormones, but the concentrations are usually subnormal and patients may be hypothyroid (28,29). Hashimoto's thyroiditis (30) and papillary carcinoma (31) have also been described.…”

“…ETT can be seen at any localization on the middle neck line, from the tongue root to the mediastinum [4][5][6]. Its prevalence is approximately 1/100.000-1/300.000.…”

“…Thin-needle aspiration biopsy was performed in order to help preoperative diagnosis and its result revealed intensive colloid and mostly isolated rare thyrocid groups (Figure 3). The patient's ectopic thyroid tissue was totally excised by dissecting it from the surrounding tissues under general anesthesia ( Figure 4,5). No complications were seen postoperatively and the histopathology results indicated nodular goiter rich in colloid, showing different sizes, with follicles containing single file epithelium, and displaying no findings in favor of malignancy ( Figure 6).…”

“…Of all ectopic thyroids 90% are found to be lingual thyroids [4]. Very rarely, two ectopic foci may be present simultaneously [5,6]. Approximately two thirds of the patients with lingual thyroid lack thyroid tissue in neck.…”
