Objective. To investigate the curative effect of guided tissue regeneration (GTR) combined with bone grafting and improve the aesthetic appearance of patients’ gingiva. Methods. A total of 86 patients with periodontal intraosseous defects were selected from February 2019 to February 2021. All the patients were divided into a control group and an observation group according to the random number table, with 43 patients in each group. Bone grafting was performed in the control group, while GTR was additionally used in the observation group on the basis of the control group. The surgical data and follow-up data were collected and organized. The alveolar bone mineral density, the change in the height of the bone defect, plaque index (PLI), sulcus hemorrhage index (SBI), PD, gingival recession (GR), clinical attachment loss (CAL), and other relevant data of the two groups in 6 months before and after surgery were compared. Six months after surgery, the cosmetic morphology of the patient's gums in the soft tissues around her teeth was evaluated. Results. Six months after surgery, the alveolar bone density of patients in two groups increased compared with that before surgery, and the height of the bone defect decreased compared with that before surgery. The alveolar bone density of the observation group was higher than that of the control group, and the height of the bone defect was lower than that of the control group ( P < 0.05 ). Six months after surgery, the PLI, SBI, PD, and CAL of patients in both groups were lower than those before surgery, while the GR was higher than that before surgery. PD and CAL values in the observation group were lower than those in the control group, and GR was higher than that in the control group ( P < 0.05 ). Six months after surgery, there was no significant difference in PLI and SBI scores between the two groups ( P < 0.05 ). Six months after surgery, the gingival cosmetic scores of the two groups of patients were higher than those before surgery. The observation group was higher than the control group ( P < 0.05 ). Conclusion. GTR combined with bone grafting has a good effect in the repair of periodontal intraosseous defects and can effectively promote the reconstruction and recovery of periodontal intraosseous defects in patients. At the same time, it can significantly improve the aesthetic appearance of patients’ gums, with good clinical application value.

Platelet-Derived Growth Factor-Modulated Guided Tissue Regeneration with a Bioresorbable Membrane in Class III Furcation Defects: A Histometric Study in the Monkey

It was the aim of this study to histometrically evaluate guided tissue regeneration (bioresorbable membrane plus bone mineral) (GTR) with or without platelet-derived growth factor (PDGF) in two different types of class III furcation defects (small keyhole defects and horizonal defects) in monkeys. In six cynomolgus monkeys, two types of class III furcation defects were created and allowed to chronify for 5 months in mandibular first and second molars. After a hygiene program the molars were assigned to GTR group (collagen membrane plus bovine bone mineral), PDGF group (collagen membrane plus bovine bone mineral plus PDGF), or negative control group (flap reposition only). Histologic sections were made after 7 months of healing and descriptive statistics were provided from the histometric parameters. Postoperative healing was uneventful despite marginal membrane exposures in the GTR and PDGF group. Bone regeneration of 23–35% of the original defect area was found in the two treatment groups. In none of the evaluated key parameters (formation of bone, root cementum, connective tissue, or epithelium) differences were detected between GTR and PDGF groups. However, the negative control teeth exhibited better bone regeneration than the treatment groups. The type of class III defect did not influence the regenerative outcome. Within the limits of this study PDGF was not able to enhance the histologic regeneration of class III furcation areas in monkeys compared to bone mineral enhanced GTR treatment regardless of the defect configuration. Membrane exposure during early healing might have influenced these outcomes.

Epidemiology and Etiopathogenetic Factors of the Development of Aggressive Forms of Periodontitis

The purpose of the work was to study the etiology and main etiopathological factors of the development of aggressive forms of periodontitis based on the analysis of additional pathogenetic aspects of mineral metabolism disorders according to modern literature. Materials and methods. The bibliosemantic method was used to clarify the state of the problem, study the analysis of the results of previous scientific research based on literature sources and electronic resources. Results. Periodontitis is the most widespread disease among all periodontal pathologies (chronic generalized periodontitis occurs in 90% of cases). Until recently, aggressive forms of periodontitis were not given due attention, since according to epidemiological studies by S. Renvert and M. Wikstom (1996), in the general structure of periodontal diseases, aggressive forms of periodontitis occur with a frequency of 5–10%. To understand the mechanisms of the development of aggressive forms of periodontitis, attention is paid to their development in children and adolescents. Clinical and radiological examinations of children and adolescents showed different frequency of aggressive forms of periodontitis (localized juvenile periodontitis and generalized juvenile periodontitis): in the USA – 2.27%, in Denmark – 0.1%, in Finland – 0.1%, in Nigeria – 0.8%, in Brazil – 0.3%, in England – 0.17%, in Chile – 0.32%. Studies have shown that aggressive forms of periodontitis are equally common in children and adolescents, regardless of gender. At the same time, localized juvenile periodontitis is 3 times more common in boys, not the same for girls. A new and rather promising vector of periodontology is the study of the role of juvenile changes in the bone skeleton on the development and course of periodontal diseases. The development of mineral metabolism disorders in aggressive forms of periodontitis in different age groups requires further study, while the data obtained by molecular geneticists become relevant for understanding the possible ways of research. Considering the fact that the jaw bones are a component of the skeleton, the strength of which can be affected by various exogenous and endogenous factors, in particular genetic ones, the main direction of searching for candidate sensitivity genes is conducted among the markers of bone metabolism disorders. Of many candidate genes that determine bone mineral density, the most well-known is the vitamin D receptor gene. Conclusion. The analysis of the available literature showed that the study of the role of mineral and bone metabolism disorders in the emergence and development of aggressive forms of periodontitis has a wide scientific perspective, and obtaining new data on the state of systemic regulatory factors and their effects on the local mechanisms of periodontal tissue damage will allow not only to improve the diagnosis of various forms, but also to increase the effectiveness of the treatment of this pathology
