Early intravenous cannulation in children during sevoflurane induction

Abstract: Following an inhalation induction with sevoflurane in children, movement with intravenous placement was greater, and the incidence of laryngospasm was higher, when the intravenous access was attempted 30 s rather than 120 s following loss of lid reflex. We recommend waiting two min following the loss of lid reflex before attempting intravenous placement in children receiving an inhalation induction with sevoflurane.

“…Movement has been reported during sevoflurane anesthesia when i.v. access was attempted (28). Two factors increase the probability of movement in response to stimulation at induction of anesthesia: early puncture of the skin and limited depth of anesthesia.…”

“…Also, during sevoflurane induction of anesthesia, it has been recommended to insert the i.v. line 2 min after the loss of lid reflex to ensure an adequate level of anesthesia and to decrease the incidence of laryngospasm (47). Laryngoscopy and tracheal intubation should also be attempted after deepening the level of anesthesia in order to avoid laryngospasm.…”

“…Additionally, parents and children often report discomfort from multiple needle marks and bruises. One study described the number of intravenous attempts observed during mask induction with sevoflurane; however, clear fluid fasting time was not considered in the analysis . Other studies have reported factors associated with difficult vascular access outside the operating room; however, these studies did not include anesthetized children .…”
