Early Clinical and Laboratory Indicators of Acute Dengue Illness

Abstract: A prospective observational study was conducted to identify early indicators of acute dengue virus infection. Children with fever for <72 h without obvious cause were studied at hospitals in Bangkok and Kamphaeng Phet, Thailand, until resolution of fever. Of 172 evaluable subjects (91% of enrollees), 60 (35%) had dengue, including 32 with dengue fever (DF) and 28 with dengue hemorrhagic fever (DHF). At enrollment, children with dengue were more likely than children with other febrile illnesses (OFI) to report … Show more

“…Prolonged thrombin time reflects alterations in fibrinogen levels and / or fibrinogen / fibrin degradation products [41]. Dengue virus infected patients frequently develop hepatic inflammation that is reflected by higher activity of hepatic transaminases AST/ALT [42], and sST2 levels were higher in patients with higher AST/ALT activity. We do not know which cells are responsible for the production of the sST2 protein in vivo during dengue virus infections; preliminary results using quantitative RT-PCR (Low Density Arrays) have shown increased expression of ST2 mRNA in peripheral blood mononuclear cells from dengue infected patients compared to OFI (data not shown).…”

“…[2][3][4] Dengue is an icosahedral, enveloped virus with a single-stranded positive sense genome; it is a member of the flaviviridae family and has 4 antigenically distinct serotypes (DENV1-4). 3,5,6 After infection of a susceptible host, an acute, selflimiting febrile systemic syndrome ensues. Resolution of infection occurs within 4-7 days and is associated with a robust innate and adaptive immune response.…”
