A rare neurological condition known as Dyke-Davidoff-Masson syndrome (DDMS) is characterized by cerebral hemiatrophy, facial asymmetry, contralateral motor impairment, and seizures. Cerebral hypoplasia, ventriculomegaly, paranasal sinus hyper-pneumatization, and compensatory osseous expansion are classic imaging findings. The relationship between clinical and neuroimaging findings provides the basis for the diagnosis of DDMS. DDMS is treated with anticonvulsant medication and physical therapy. This is the case of a 26-year-old male, with a known case of cerebral palsy and on antiepileptics for 15 years, who presented with complaints of generalized weakness, vomiting, difficulty walking, recurrent convulsions, and a recent episode of generalized tonic-clonic seizures. Neuroimaging revealed findings significant of Dyke Davidoff Masson Syndrome. Understanding its imaging and clinical characteristics facilitates prompt and precise diagnosis, permitting appropriate management.

Background:The first examples of this syndrome were identified and described by Dyke, Davidoff and Masson in 1933. The global incidence of the condition is still uncertain, even though 100 cases have been reported worldwide almost a century after its discovery. Clinical symptoms include mental retardation, hemiparesis or hemiplegia, facial or bodily asymmetry with atrophy and localised and/or widespread drug-resistant epilepsy. Case description: 23-year-old G2A1(gravida 2 abortion 1) presented with history of amenorrhoea since 9 months and pain in abdomen since 6 hours and she late developed complex partial status epilepticus and had a history of recurrent seizures since 10 years of age. MRI brain was done which revealed Dyke Davidoff Masson Syndrome. Conclusion: DDMS, a condition characterized by frequent and persistent seizures, is rare and can be difficult for medical professionals to diagnose accurately due to its rarity. However, specific imaging findings related to this disease can be revealed using MRI.

Dyke–Davidoff–Mason syndrome is an extremely rare medical condition that results from brain damage occurring during foetal life or early childhood. The aetiology of the syndrome can be congenital or acquired. The clinical presentation of the syndrome is variable, and depends on the extent of damage to the central nervous system. It most commonly manifests as epileptic seizures, intellectual disability, and hemiparesis/hemiplegia, which is usually preceded by the first seizure. It is suspected that intellectual impairment is a consequence of epilepsy. The spectrum of symptoms also includes psychiatric conditions, particularly psychotic disorders and mood disorders. Radiological manifestations of the syndrome include midline cerebral shift, widening of the Sylvian fissure, hemispheric aplasia/hypoplasia of the brain, ipsilateral cranial bone thickening, lateral ventricle enlargement, as well as hyperpneumatisation of the frontal and sphenoid bones, and the mastoid process. Diagnosis is mainly based on imaging tests such as computed tomography and magnetic resonance imaging, as well as neurological examination and electroencephalography. During the patient’s history-taking, particular attention should be paid to pregnancy history and the childhood period in order to identify potential etiological factors. No characteristic changes in laboratory tests indicating Dyke–Davidoff–Mason syndrome have been found. Treatment is primarily symptomatic. The greatest challenge for physicians is the control of epileptic seizures. Conservative treatment is initially employed. If proven to be ineffective, surgical treatment should be considered.
