Duodenal Adenocarcinoma: Why the Extreme Rarity of Duodenal Bulb Primary Tumors?

Abstract: Adenocarcinoma of the small bowel accounts for only one per cent of all gastrointestinal malignancies. Duodenal adenocarcinoma accounts for half of all small bowel adenocarcinomas. The duodenum is divided into four segments: D1 (proximal horizontal 5 cm beginning with the 3-cm duodenal bulb), D2 (descending), D3 (distal horizontal), and D4 (ascending). The most common location of duodenal adenocarcinomas is the ampullary region of D2. Based on observational experience, our hypothesis was that primary adenocarc… Show more

“…Except for carcinoma of the ampulla of Vater, primary duodenal adenocarcinomas are rare and account for only 0.3% of carcinomas of the digestive tract [1] . In recent years, the increased use of endoscopies has led to an increased identification of duodenal tumors [2] ; however, a neoplasm in the third or fourth portion of the duodenum is an extremely rare entity and is sometimes difficult to treat [3] . Endoscopic treatment is generally appropriate for dysplastic polyps or intramucosal adenocarcinomas, although some cases may be difficult to resect endoscopically because of the thinness of the duodenal wall and the narrow working space [4] .…”

“…Первичные злокачественные опухоли чаще всего локализуются в нисходящем отделе ДПК, режев нижнегоризонтальном сегменте и в дуоденоеюнальном переходе. В начальном отделе ДПК злокачественные опухоли встречаются крайне редко [6][7][8].…”

“…The ileum carries the majority of the small intestinal tumor burden, followed by the duodenum, and lastly the jejunum [ 1 ]. The majority of duodenal cancers originate from the descending duodenal segment (D2), followed by the horizontal (D3) and ascending (D4) segments, and rarely from the proximal horizontal segment (D3) [ 2 ]. More than 40 histological subtypes of small intestinal malignancies have been described, the most common being adenocarcinoma, sarcoma, lymphoma, and neuroendocrine tumors [ 3 ].…”
