Drug Reaction To Ursodeoxycholic Acid: Lichenoid Drug Eruption In An Infant Using Ursodeoxycholic Acid For Neonatal Hepatitis

Abstract: Lichenoid drug eruptions are skin reactions associated with a number of drugs and chemicals. They are rare in infants. We report here an infant who presented with neonatal hepatitis and who was being treated with ursodeoxycholic acid. The infant developed lichenoid skin eruptions in the third week of the therapy. The lesions have resolved two months after the discontinuation of the drug. To our knowledge, this is the first infant case of lichenoid drug eruption associated with ursodeoxycholic acid.

“…Also, vaccines have been reported to cause LDE in a total of 8 (2.5%) cases (hepatitis A/B, influenza, herpes zoster, human papillomavirus) 115–119 Four cases (1.24%) of Capecitabine‐induced LDE were reported 120–123 . Two cases (0.62%) for each of the following drugs were identified: Thioridazine, leflunomide, ibuprofen, sildenafil and ursodeoxycholic acid 124–133 . Single cases of a large number of additional drugs were identified as listed in Table S1 33,77,86,134–169 …”

“…A skin eruption initially attributed to UDCA was caused by allergy to a co‐ingredient, as shown by lymphocyte stimulating tests 90 . A lichenoid skin eruption was reported in an infant with neonatal hepatitis treated with UDCA, but the component of the UDCA preparation responsible for the skin reaction remains unclear 91 …”

“…Pediatric LDE is infrequent due to the relative uncommon use of systemic medications in this age group. A review of the current literature revealed several cases of cutaneous LDE in children (28,29). However, reports of OLDE are extremely rare and may be related to lack of recognition or misdiagnosis as other clinical entities, including idiopathic LP.…”
