“…Drug induced RPF is a rarely reported complication in subjects following the usage of ergot alkaloids (eg, ergotamine, methysergide). 18 A previous report described a distinctive restrictive type pleuropulmonary pattern of fibrosis following long-term therapy with ergot derivatives (nicergoline, dihydroergocristine, dihydroergotamine), as opposed to typical interstitial pattern observed in other drugs. None of the cases were associated with either clinical or biological evidence of a systemic illness such as rheumatoid arthritis, ankylosing spondylitis, mixed connective tissue disease, lupus erythematosus, or inflammatory bowel disease, and vasculitic mechanism was not described in any of those cases.…”

“…MTX is now considered a “possible” inductive factor for retroperitoneal fibrosis (RPF). Reports of MTX causing RPF are extremely infrequent making the description of the pathogenesis elusive [ 22 ]. Paradoxically, there is (limited) data suggesting that methotrexate may be effective in combination with prednisone for the treatment of relapsing idiopathic retroperitoneal fibrosis [ 23 ].…”

“…This increased serotonin via feedback mediates myofibroblast proliferation with overproduction of collagen, fibronectin, tenascin and glycosaminoglycans. 8 From our literature search, no published human studies were found exploring the effect of ergotamine on the dura mater. We postulate whether a similar rebound release of serotonin after ergotamine use could be the pathogenesis of pachymeningitis, similar to what is hypothesised to occur in other organs.…”
