The term “spaces” refers to the areas delimited by at least two independent fasciae and filled with areolar connective tissue. However, there is discrepancy regarding the spaces and their limits between clinical anatomy and gynecologic surgery, as not every avascular space described in literature is delimited by at least two fasciae. Moreover, new spaces and surgical planes have been developed after the adoption of laparoscopy and nerve-sparing gynecological procedures. Avascular spaces are useful anatomical landmarks in retroperitoneal anatomic and pelvic surgery for both malignant and benign conditions. A noteworthy fact is that for various gynecological diseases, there are different approaches to the avascular spaces of the female pelvis. This is a significant difference, which is best demonstrated by dissection of these spaces for gynecological, urogynecological, and oncogynecological operations. Thorough knowledge regarding pelvic anatomy of these spaces is vital to minimize morbidity and mortality. In this article, we defined nine avascular female pelvic spaces—their boundaries, different approaches, attention during dissection, and applications in obstetrics and gynecology. We described the fourth space and separate the paravesical and pararectal space, as nerve-sparing gynecological procedures request a precise understanding of retroperitoneal spaces.

Цель исследования -проанализировать результаты современных научных исследований, касающихся анатомии фасций и клетчаточных пространств подбрюшинного этажа женского таза. В литературном обзоре выполнен анализ современных российских и зарубежных (исключительно на английском языке) научных работ, значимых относительно темы обзора и представленных в базах данных eLibrary, PubMed, Scopus и в научной электронной библиотеке «КиберЛенинка» (Cyberleninka). В обзоре представлены современные данные относительно границ и содержимого каждого клетчаточного пространства подбрюшинного этажа женского таза. Указана клиническая значимость межфасциальных пространств в качестве хирургических доступов. Количество пространств подбрюшинного этажа женского таза, по данным различных авторов, варьирует от шести до девяти. Эти пространства друг от друга отделяют три пары связок: кардинальные связки матки, крестцово-маточные и пузырноматочные связки. Названия пространств происходят от наименований органов, окружающих их. По топографическому принципу все пространства разделяют на медиальные и латеральные. К латеральным пространствам относят околопузырное, околопрямокишечное пространства, а также пространство Ябуки. Медиальные пространства включают в себя позадилобковое пространство, пузырно-маточно-влагалищное пространство, прямокишечно-влагалищное и позадипрямокишечное пространства. Ключевые слова: клетчаточные пространства, подбрюшинный этаж таза, пространство Окабаяши, пространство Лацко, предпузырная фасция, позадипузырная фасция, околопузырное клетчаточное пространство.

A Bipartite Obturator Artery with Multiple Pelvic Branching—A Gynecologic Approach

Background: The obturator artery (OA) often presents multiple anatomical variations. These can be an atypical origin, variable anastomosis, or abnormal course within the pelvis. Methods: This study aimed to report a rare arterial variation in a Puerto Rican female cadaver that showed two abnormal obturator arteries with multiple pelvic branches. The OA emerged from the anterior branch of the internal iliac artery, which typically runs anteroinferior along the lateral wall of the pelvis to the upper part of the obturator foramen. Results: The atypical OA described in this report provided two variant branches. Abnormal obturator artery I (AOAI) emerged first and gave rise to three additional branches, while abnormal obturator artery II (AOAII) emerged second and gave rise to two other branches. Conclusions: Identifying these accessory arteries is essential for surgical interventions, particularly within the field of gynecology and urogynecology. Knowledge regarding anatomical variations within this region must be assessed preoperatively to decrease the risk of iatrogenic injury.
