Double-Blind Randomized Trial of ACTH versus Dexamethasone versus Methylprednisolone in Multiple Sclerosis Bouts

Abstract: 30 patients with acute exacerbations of multiple sclerosis were treated by ACTH, dexamethasone or methylprednisolone in a double-blind randomized study. Clinical parameters were assessed; cerebrospinal fluid and neurophysiological parameters (visual- and brainstem-evoked potentials) were evaluated at the beginning and at the end of treatment. Dexamethasone was more effective than ACTH and 6-methylprednisolone in shortening bout duration. Neither CSF nor neurophysiological parameters were significantly affected… Show more

“…Additionally, daily administration of 500-1000 mg of MP increases serum steroid peak levels 5-10 times more compared with ACTH administration (54). Despite these, positive effects equivalent to MP have been reported with ACTH use in relapse treatment (59,60,61). This effect is attributed to the direct anti-inflammatory and immunomodulatory effects of ACTH through melanocortin receptors.…”

“…Steroids are widely used in the treatment of MS exacerbations, but is still uncertain if their mechanisms of action are mainly antiedemic [14], antiinflammatory [6] or something else [3,29]. In a previous study, we demonstrated that dexamethazone (DX) is more effective that ACTH or low-dose methylprednisolone (LDMP) in bout therapy [19]. No correlation has been found between clinical efficacy and short-term changes in visual (VEPs) or brain-stem auditory (BAEPs) EPs [26].…”
