Background: Donor notification is emphasized as an efficient method of curtailing TTIs recently. But its limitations like low notification rate and lack of follow up of treatment of notified donors are still unaddressed. The aim of the study was to analyze the response rate of notified reactive donors, to elicit hidden risks factors and to see impact of donor notification on reactive donors.Methods: This retrospective study was conducted in the blood bank of a 350 bedded multispecialty hospital in north India. Data was collected from reactive blood donor counselling register over a period of one year. After six months a telephonic interview was conducted of all the reactive donors who responded to notification calls and came for counselling to inquire if they went for further testing and treatment after they were notified.Results: There were 1345 whole blood donations over a period of 14 months of which 29 (2.15%) were reactive donors. Notification rate was 48.27%. During counselling 4 donors revealed high risk history which they had denied during pre-donation counselling. After telephonic conversation with these notified donors, we analyzed that 7 (50%) donors were taking treatment while 3 donors informed that they got themselves tested from a private lab and were reported negative. Two donors informed that they had not gone for any testing anywhere while we could not trace 2 donors due to change of contact numbers.Conclusions: Curtailing TTI’s through donor notification by blood banks alone is an unattainable mission. A centralized computer data system connecting all blood banks and interlinking of government agencies and blood banks like the recently started NVHCP is need of the hour.

Background: Blood transfusion plays a vital role in the management of many diseases but with risk of TTI transmission & many adverse reactions. Blood donor screening and transfusion transmitted infections testing ensures blood safety, so become more stringent all over the world. Aims & objective: The main aim of this study was to assess the response rate of sero-reactive donors. Material and Methods: It was an observational and prospective study done in our department for a period of 13 months during which response rate of TTI reactive donors was analyzed from reactive donor registry. Results: In this study, total 7901 units were screened to TTI screening test. Out of which 130 units (1.6%) were found to be seroreactive. Out of 130 reactive donors, 90 (69.2%) donors were contacted and only 51 (56.6%) donors responded to call & attended counselling and referral to other department for treatment. Conclusion: Universal guidelines, protocols and confidentiality is maintained by every blood center for donor notification. Response rate of reactive donors helps us to frame guidelines to track non responding donors who pose major threat to the healthy donor pool.

Background: As blood transfusion is associated with risk of transmitting transfusion transmitted infections (TTI), another tool to prevent it is to inform, notify and counsel the donors about their seroreactive status at the blood centre. Methods: A retrospective study was carried out in department of IHBT, at BMC from January 2017 to October 2019. The reactive donors were informed by the blood bank counselor about an abnormal test result with an advice to report to the blood bank for one to one counseling and repeat testing, as well as for referral to the respective department or Integrated counseling and testing centre or sexually transmitted disease clinics of the hospital for further management by means of telephonic calls, letters and personal interrogation. The response rate of TTI reactive donors after notification was evaluated. Results: There were total 41,114 blood donation over the period of almost 3 years out of which 617 were reactive donors (1.5%) with median age of 31.5 years and female to male proportion of 7:610. History of tattooing for more than 1 year was present in 155 donors. Amongst reactive donors 336 were voluntary while 281 were replacement donors. Seropositivity for HIV, HBV and Syphilis was more in replacement donor while HCV was more in voluntary donors. HBV, HCV, Syphilis and HIV were 316(0.87%), 136(0.33%), 69(0.17%) and 56(0.13%) respectively in reference to total population. There were 4 cases of co-infection 2 for HIV+HBV, 1 for HBV + Syphilis and 1 for HIV+HCV. All donors were contacted telephonically. Inaccurate donor demographics (wrong address and phone number) resulted in our inability to notify 6 (0.97%) cases. Out of 56 HIV reactive donors 26 were voluntary and 30 were replacement donors and response rate for post donation counseling was 98%, 4 HIV donors were previously registered at ART centre and 4 became non reactive so they were again followed up after 3 months. Conclusion: A large majority of the notified donors in our study contacted their health care provider when given clear instructions to do so. Stringent notification data on HBV, HCV and Syphilis reactive donors is required. Thorough tracking system for follow up of reactive donors is required.
