Background: The third stage of labor is a critical period, thus using high fidelity simulation to enhance intern students' competency through implementing the clinical guidelines for active management of the third stage of labor is essential for preventing its serious complications mainly postpartum hemorrhage. The aim of this study: Was to determine the effect of high fidelity simulation on intern students' competency regarding clinical guidelines for active management of the third stage of labor. Subjects and Method: The study was carried out at Objective Structure Clinical Examination (OSCE) skills laboratory at Faculty of Nursing, Tanta University. A convenient sample consists of 50 intern nursing students who were trained at labor unit, obstetric department, Tanta university hospital and fulfilling the inclusive criteria. Three tools were used to collect the data: Tool (I): A structured interview schedule included three parts: Part (a) Socio demographic characteristics of the intern nursing students, part (b) Intern students' knowledge regarding the third stage of labor, and part (c) Intern students' knowledge regarding clinical guidelines for active management of the third stage of labor. Tool (II): Intern students' practice observational checklist contained seven sequential steps of clinical guidelines for active management of the third stage of labor. Tool (III): Modified Self-Confidence Measurement Scale included 7 items that measured how confident intern nursing students felt about the skills they performed during simulation training program. Conclusion: Based on the findings of the present study, it can be concluded that high fidelity simulation training significantly improved the intern students' knowledge, practice as well as the self-confidence regarding clinical guidelines for active management of the third stage of labor. Therefore, the study recommended that providing pre-service and in-service training programs using high fidelity simulation especially for newly appointed intern nurses is crucial to improve their knowledge and practices as well as the self-confidence.

Introduction Maternal mortality is a health problem in developing countries and is the result of several factors such as sociodemographic and economic inequalities and difficulties in accessing the health services. In addition, training strategies in obstetric emergencies targeting the non-medical personnel such as traditional midwives are scarce. The focus of this study is to develop learning and communication bridges on the management of obstetric emergencies and on policies of patients’ referral to the biomedical health system in rural areas. Methodology A Participant Action Research (PAR) study with a mixed methods approach was set up to elaborate culturally adapted flowcharts. The project lasted approximately 3,5 years, from September 2016 to January 2021. Results The study was conducted with 94 traditional midwives from southern Ecuador and is divided into 4 phases, namely: 1) Exploration: focus groups and interviews were conducted to document the management of obstetric emergencies through the presentation of “clinical case” scenarios in three important topics, namely: pre-eclampsia, shoulder dystocia and postpartum hemorrhage, 2) Planning: a number of reflective sessions were conducted between the researchers and the healers/midwives to elaborate flowcharts. 3) Action: the training was conducted in rooms dedicated to proficiency in the aforementioned topics and using the flowcharts, 4) Evaluation: 90% of the participants reported having used the flowcharts during the first year after the training. The most frequently used flowchart was that of pre-eclampsia for the recognition of warning signs during pregnancy control. Conclusion This study documents common practices of pregnancy and delivery management by traditional midwives. Furthermore, cultural flowcharts were developed for and together with midwives to improve the clinical response to obstetric emergencies. The preliminary evaluation was favorable; the most frequently used flowchart concerned preeclampsia. In this process, establishing a partnership was crucial for successful intercultural collaboration.

Background The purpose of this study was to evaluate a teaching strategy that engaged baccalaureate maternity nursing students at two universities in a postpartum hemorrhage escape room. The study examined students’ attainment of knowledge, as well as teamwork, collaboration, communication, and safety. Method A mixed method study was conducted with 85 junior-level and senior-level maternity students at two universities in the Northeastern region of the United States. Participants completed a survey and attended one of 20 focus groups after completion of a healthcare simulation escape room on postpartum hemorrhage. Results Participants (95.2%) reported that the escape room helped with knowledge and skill acquisition. All of the participants reported that it was an effective team building strategy and 97.6% said it fostered communication, learning, and the ability to provide safe patient care. Participants unanimously reported that the escape room was enjoyable, an effective strategy for team building, and they would recommend it to others. Qualitative findings showed six major themes: It was Fun!, No I in Team, Escaping to Learn: Building Knowledge and Confidence, Pressure Cooker, Big Picture, and Lessons Learned. Conclusion Healthcare simulation escape rooms are an effective teaching strategy and can be used by nursing faculty to foster knowledge, teamwork, collaboration, communication and safety. Participant feedback showed high satisfaction and they felt the escape room provided a valuable learning experience.
