Diuresis Renography in Equivocal Urinary Tract Obstruction

Abstract: A trial of the value of diuresis renography in equivocal upper urinary tract obstruction is presented. Fifty-two patients were examined by standard renography and renography under a diuretic provocation. The results indicate that the technique is of considerable value in making the vital distinction between dilatation on the excretion urogram due to atonicity and that caused by a genuine obstruction. A quantitative index of the response to diuresis reflecting the degree of impedance to flow is described--the D… Show more

“…However, this concept does not seem to be widely accepted in pediatric urology. 21 In addition, they report on patients with a significantly higher age, when compared with our series (median age, 11.3 years vs 22 months). Thus, they describe a group of patients with different anatomic preconditions that might be more suitable for placement of short-term retrievable ureteral Double-J stents.…”

“…Indications for surgery were hydronephrosis grade 3 associated with loss of DRF 10% and/or urodynamic relevant obstruction in technetium-99 m-MAG3 diuretic renography according to O'Reilly et al, 21 as well as an aberrant lower pole vessel, detected by ultrasound.…”

“…The introduction of the diuretic renogram in 1978 [28][29][30] was based on an assumption that the radionuclide retained in a dilated nonobstructed collecting system, when challenged with an increased urine flow, will be washed out. Administration of a diuretic to a dilated obstructed system will result in retention of the radio- tracer proximal to the site of obstruction [31] (Fig.…”

“…Furosemide renography is also initially performed conventionally and later supplemented by an injection of furosemide [O'Reilly et al, 1978;Koff et al, 1979;O'Reilly et al, 1979;Koff et al, 1980;O'Reilly and Lupton, 19821. If the drainage curve decreases substantially, the condition is judged as unobstructed, while mildly decreasing or unchanged isotope level is judged as partial or severe obstruction.…”
