Disulfiram Induced Psychosis

Abstract: Disulfiram is the commonly prescribed drug for the treatment of alcohol dependence. It’s major metabolite (diethyldithiocarbamate) is an inhibitor of dopamine-betahydroxylase, an enzyme that catalyzes the metabolism of dopamine to norepinephrine resulting in psychosis. We recommend that disulfiram should be used at the lowest effective dose, possibly 250 mg daily and caution should be taken while prescribing disulfiram for patients with personal and familial antecedents of psychosis.

“…By inhibiting dopamine β-hydroxylase, DSF simultaneously reduces NE and elevates DA in these tissues. A link has been established between psychosis and DSF-induced increase of DA in the mesolimbic system (10,33). Dopamine β-hydroxylase is also expressed in some peripheral sensory neurons and it has been suggested that neurotoxic products of catecholamines metabolism in nociceptors can cause neuronal dysfunction underlying neuropathic pain (34).…”

“…Disulfiram is commonly used in dosages of 250-500 mg/day. 1 Although well tolerated by most patients, severe toxic side effects have also been reported including hepatitis, encephalopathy, psychosis, optic, and peripheral neuropathy. 2 We are reporting a case of disulfiram toxicity in a 4.5-year girl who ingested 4-5 tablets of disulfiram (approximately 1-1.25 g) accidentally and presented with hypoglycemia and encephalopathy.…”

“…We read with great interest the recently published article by Mohapatra and Rath 1) regarding a case of psychosis after treatment with disulfiram. Because of our continuous research on disulfiram and its side effects, we find the issue very interesting and we would like to comment briefly on this report.…”
