Visceral leishmaniasis (VL) is a systemic disease endemic in tropical countries and transmitted through sand flies. In particular, <em>Canis familiaris</em> (or domesticated dogs) are believed to be a major urban reservoir for the parasite causing the disease Leishmania. The average number of human VL cases was 58 per year in the state of Sergipe. The city of Aracaju, capital of Sergipe in Northeastern Brazil, had 159 cases of VL in humans. Correlatively, the percentage of serologically positive dogs for leishmaniasis increased from 4.73% in 2008 to 12.69% in 2014. Thus, these studies aimed to delineate the spatial distribution and epidemiological aspects of human and canine VL as mutually supportive for increased incidence. The number of human cases of VL and the frequency of canine positive serology for VL both increased between 2008 and 2014. Spatial distribution analyses mapped areas of the city with the highest concentration of human and canine VL cases. The neighbourhoods that showed the highest disease frequency were located on the outskirts of the city and in urbanised areas or subjected to development. Exponential increase in VL-positive dogs further suggests that the disease is expanding in urban areas, where it can serve as a reservoir for transmission of dogs to humans via the sand fly vector.

Resumo: A expansão das áreas de transmissão de leishmaniose visceral (LV) traz questionamentos sobre as estratégias de controle empregadas no Brasil. As diretrizes do Programa de Vigilância e Controle da Leishmaniose Visceral (PVCLV) estão centradas na diminuição da morbidade e letalidade, controle do reservatório e dos vetores e na educação em saúde. Este artigo buscou avaliar as dificuldades na execução das ações preconizadas pelo PVCLC, segundo os seus coordenadores em municípios brasileiros de grande porte com transmissão canina e/ou humana (Campinas, Bauru, Goiânia, Campo Grande, Fortaleza e Belo Horizonte). Foram realizadas entrevistas semiestruturadas, sendo os principais problemas identificados: descontinuidade das atividades de controle, resistência dos proprietários de cães com indicação de eutanásia e baixa cobertura do controle químico. Como conclusão, as entrevistas mostraram que inúmeros fatores impedem o cumprimento das atividades propostas pelo PVCLC, ficando clara a necessidade de reavaliação da política brasileira de controle de LV.

RESUMO.-[Detecção molecular e sorológica de infecção por Leishmania (Leishmania) infantum em cavalos na região Sudeste do Brasil.] O estudo objetivou verificar a ocorrência de Leishmania spp. e Leishmania (Leishmania) infantum em cavalos de uma região endêmica para leishmaniose visceral do Brasil. Amostras de DNA de sangue e suabe conjuntival (SC) foram testadas pela PCR e pela Reação de Imunofluorescência Indireta (RIFI). Embora nenhum cavalo estivesse clinicamente doente, animais infectados por Leishmania spp. e L. (L.) infantum foram encontrados em Ilha Solteira/SP. Dos 40 cavalos, 100% (40/40) foram positivos pela PCR de sangue, 90% (36/40) pela PCR de SC, e 2,5% (01/40) no sorodiagnóstico, pela RIFI. Seis desses 40 cavalos foram positivos para L. (L.) infantum pela PCR de sangue. O sequenciamento direto e a análise dos amplicons resultaram em uma sequência para análise evolutiva. Os resultados indicam a presença de Leishmania spp. e L. (L.) infantum infectando cavalos saudáveis no Brasil. A presença de DNA de Leishmania spp. and L. (L.) infantum em cavalos saudáveis sugere que eles podem ser importantes reservatórios desses parasitas, um achado altamente relevante para a vigilância epidemiológica das doenças que causam.
