Distinguishing well-differentiated hepatocellular carcinoma from benign liver by the physical features of fine-needle aspirates

Abstract: Distinguishing well-differentiated hepatocellular carcinoma (HCC) from benign hepatic lesions is challenging for pathologists in limited diagnostic material such as needle-core tissue biopsy and fine-needle aspiration (FNA) biopsy. The objective of this study is to test a hypothesis that the fortification of liver by reticulin along single cell plates should protect benign hepatic lesions from breakdown by the force of aspiration and smearing, whereas the decreased reticulin in well-differentiated HCC would re… Show more

“…Demonstration of biliary-type CK immunostaining with monoclonal antibodies to CK7 and CK19 also supports the diagnosis of FNH because the presence of ductular structures is typically part of the histological picture of FNH [3]. Although the reticulin stain [9][10][11] and immunostaining for CD34 (endothelial antigen) [12] have been reported to help in differentiating hepatocellular carcinoma (HCC) from FNH as well as other benign lesions of the liver, in our experience, they do not help in difficult cases.…”

“…The cytotechnologist made Swedish-style oval smears with "head, body, and tail." 12,13 One smear was plunged immediately into a coplin jar containing 95% ethanol for traditional Papanicolaou staining. The remaining smears were air dried.…”

“…8,11,12 The absence of eccentric nuclei, anisokaryosis, and irregular nuclear contours or the presence of small numbers of naked nuclei might contribute to the differentiation of HCA from these lesions. HCC and DN: Several articles have discussed ways of cytologically distinguishing between HCA and these lesions.…”
