Dissection of the Abdominal Aorta. Current Evidence and Implications for Treatment Strategies: A Review and Meta-Analysis of 92 Patients

Abstract: AAD is a rare event that appears to be associated with hypertension and pre-existing aneurysmal degeneration of the abdominal aorta. AAD patients are at considerable risk of in-hospital mortality and complications. Endovascular therapy appears to be associated with a relatively low risk of mortality or major complications compared to open repair and conservative treatment.

“…Medically managed IAADs usually progress with a high mortality rate; therefore, aggressive surgical or endovascular treatment has been recommended. 4,5 Although endovascular treatment of infrarenal IAAD has recently been reported as an efficient therapeutic approach, 6,7 whether endovascular treatment of suprarenal IAAD is safe with a durable result is unknown. Successful endovascular treatment of aortic dissection could promote aortic remodeling, but little has been reported about aortic remodeling after endovascular intervention in IAADs.…”

“…In cases of true abdominal location the entry is located between the renal arteries and the inferior mesenteric artery (in 50% of cases), while re-entry is usually in the CIA [10].…”

“…Endovascular methods are often the first line treatment nowadays, and are especially effective for long lesions [10,14]. The aim of the therapy is implantation of a stent or stentgraft into the true lumen with the aim of collapsing the false lumen and redirecting the blood flow to the true lumen.…”

“…The results from a meta-analysis published by Jonker et al 8 suggest that an early aggressive (open or endovascular surgery) may be associated with improved survival. However this is a retrospective study with only 19 patients treated with endografts, therefore, in our institution, we still manage the AIAAD with medical treatment and treat these patients in case of ischemic complications.…”
