Acanthamoeba keratitis is a rare but serious complication of contact lens wear that may cause severe visual loss. The clinical picture is usually characterised by severe pain, sometimes disproportionate to the signs, with an early superficial keratitis that is often misdiagnosed as herpes simplex virus (HSV) keratitis. Advanced stages of the infection are usually characterised by central corneal epithelial loss and marked stromal opacification with subsequent loss of vision. In this paper, six cases of contact lens-related Acanthamoeba keratitis that occurred in Australia and New Zealand over a three-year period are described. Three of the patients were disposable soft lens wearers, two were hybrid lens wearers and one was a rigid gas permeable lens wearer. For all six cases, the risk factors for Acanthamoeba keratitis were contact lens wear with inappropriate or ineffective lens maintenance and exposure of the contact lenses to tap or other sources of water. All six patients responded well to medical therapy that involved topical use of appropriate therapeutic agents, most commonly polyhexamethylene biguanide and propamidine isethionate, although two of the patients also subsequently underwent deep lamellar keratoplasty due to residual corneal surface irregularity and stromal scarring. Despite the significant advances that have been made in the medical therapy of Acanthamoeba keratitis over the past 10 years, prevention remains the best treatment and patients who wear contact lenses must be thoroughly educated about the proper use and care of the lenses. In particular, exposure of the contact lenses to tap water or other sources of water should be avoided.

Adhesion of bacteria to hydrogel lenses is thought to be an initial step of ocular colonization allowing evasion of normal host defences. The salt concentration of media is an important parameter controlling microbial adhesion. Salinity varies from 0.97% NaCl equivalents in the open eye to 0.89% in the closed eye state. In this study, the effect of sodium chloride in the concentration range of 0.8-1.0% (w/v) NaCl on adhesion of ocular bacteria to soft contact lenses was investigated using a static adhesion assay. Pseudomonas aeruginosa was found to adhere to lenses in significantly greater amounts than Serratia marcescens, Flavobacterium meningosepticum, Stenotrophomonas maltophilia and Staphylococcus intermedius. Increasing NaCl from 0.8% to 1.0% (w/v) increased adhesion of all bacteria tested. This adhesion was strong since the organisms could not be removed by washing in low ionic buffer. Adhesion of these organisms did not correlate with their cell surface properties as determined by bacterial adhesion to hydrocarbons (BATH) and retention on sepharose columns.

The present study aims towards investigating the risk of developing complications upon usage of different types of contact lens as well as the role played by different variables related to contact lens usage (replacement time, regular follow-up, etc). This is a retrospective cohort study. The total study sample (n = 143) were recruited from Rafha General Hospital, Northern Border Region, Saudi Arabia. All their data were collected and analyzed using chi square test. A P-Value of <0.05 was considered statistically significant. Reusable contact lens wearers were noted to have significantly more complications than disposable contact lens wearers (P<0.05). The majority of the sample was reported to use contact lens for cosmetic (decorative) purpose than for therapeutic or optical indications (63%). Regular follow-up was significantly associated with decrease incidence of contact lens complications (P<0.01). Handling of reusable contact lenses must be done with care as complications are more likely to occur by using them than daily disposable contact lenses. Moreover, contact lens wearers must be advised about the importance of frequent replacement and regular follow-up as it showed significant decrease in complications. We can conclude from our finding that cosmetic usage of reusable contact lenses without proper handling and cleaning system is not advisable.
