Discrepancies Between Venography and Real Time B Mode Ultrasound Imaging in the Diagnosis of Deep Vein Thrombosis

Abstract: Methods : in this prospective study, real time B Mode ultrasound imaging (USI) was compared to bilateral ascending contrast venography, double blindly, in 430 patients suspected of deep vein thrombosis (DVT) or pulmonary embolism.A complete scan of the venous system from the inferior vena cava to the calf veins, was performed with a high resolution duplex system (DIASONICS DRF 400) and coupled systematically with a C.W. Doppler examination. The results obtained by USI were thus compared to the venograms perfor… Show more

“…Compression ultrasound is also a valid diagnostic modality in isolated calf vein thrombosis, which is detected with sensitivities and specificities of 85-90% (see Table 3.6; Elias et al 1987;). Compression ultrasound is also a valid diagnostic modality in isolated calf vein thrombosis, which is detected with sensitivities and specificities of 85-90% (see Table 3.6; Elias et al 1987;).…”

“…Elias et al. [5] were the only ones to present early accuracy data as well as later outcome studies, thereby documenting their detailed interest in distal ultrasound. Accordingly, in a large validation cohort they found sensitivity and specificity of 98% and 95%, respectively.…”

“…The diagnosis of DVT in the popliteal, femoral, and iliac veins was documented by ultrasonography 16,17 or by venography. Patients were excluded if they had a thrombus located in the inferior vena cava, a floating thrombus, a history of DVT within the preceding 6 months, DVT with an objectively documented symptomatic pulmonary embolism, a clinical context needing hospitalization, or a contraindication to anticoagulant treatment (short-or long-term anticoagulants); if they received treatment with heparin (other than prophylactic) within the 48 hours preceding inclusion; if they were pregnant; if home treatment or a hospital stay was impossible; if they lived too far from the center; or if they would not give written consent.…”
