Digital Subtraction Angiography for CT Angiogram Negative Haemorrhages

Abstract: In patients with SAH, negative findings on a technically sound CTA are reliable in ruling out aneurysms in any pattern of SAH or no blood on CT. Our observations need to be confirmed with larger prospective studies.

“…The non-invasiveness and short acquisition time of CTA give it more advantages for acute bleeding and a higher priority than DSA (18,19). However, some literature points out that CTA may have false-negative results which can occur if the bleeding is not obvious at the time of the scan due to the short acquisition time (19)(20)(21). Considering that the patient had no indication of active bleeding, and it was difficult to locate the culprit vessel using CTA, we did not perform CTA in this case.…”

“…The rate at which DSA detects the cause of bleeding in cases of CTA-negative SAH is 4–8%. [ 1 , 20 ] Although there are few patients with SAH who have negative findings on CTA and positive findings on DSA, it is difficult to distinguish small perforating arteries of less than 1 mm in diameter or to detect very small aneurysms on CTA. [ 3 ] Therefore, even if CTA findings are negative for the cause of SAH, DSA is necessary to investigate the cause of bleeding.…”

“…It is less time-consuming, cheaper and facilitates scanning patients with pacemakers or other metals. Another feature of CTA is that it provides the best images which show the relationship between aneurysms, vessels, and bone structures [13,14]. CTA was performed first in our patients due to its easiness, cost-effectiveness, and ability to show the relationship with bony structures.…”
