Digital Breast Tomosynthesis–guided Vacuum-assisted Breast Biopsy: Initial Experiences and Comparison with Prone Stereotactic Vacuum-assisted Biopsy

Abstract: Clinical performance of DBT VAB was significantly superior to PS VAB. Because DBT VAB allows use of the full detector size for imaging and provides immediate lesion depth information without requiring triangulation, it facilitates target lesion reidentification and sampling of even low-contrast targets, such as uncalcified masses.

“…2,7 It has also been suggested that DBT may replace prone stereotactic vacuum-assisted breast biopsy (VABB) and that DBT-guided biopsy is likely to expand the number of mammographic lesions considered amenable to mammographic VABB. 8 Consequently, percutaneous core needle biopsies on DBT-detected lesions are going to increase. Percutaneous core needle biopsies are associated with pain and emotional distress, and there are some differences in the techniques related to VABB performed on DBT or digital mammography (MMx).…”

“…21,22 Such devices range from 7-12 G and can be used to remove tissue volumes equivalent to the weight of a surgical specimen. This allows the pathologist considerably more tissue for analysis improving diagnostic accuracy, but requires additional processing and reporting time to ensure the sample has been sufficiently scrutinized.…”

“…Therefore, DBT may have an important role to play in the sampling of such tissues. Furthermore, DBT has the potential to assist with not only accurate lesion localisation but also needle path planning due to its 3D capabilities. Few studies have focused on evaluating DBT in a biopsy scenario.…”

“…One study with a sample size of 205 women reported a 100% success rate in tissue sampling with DBT‐guided VAB, with 43% reduction in procedural time, with fewer acquisitions and without causing any complications as compared to mammography‐guided VAB . Another recent study has seconded these results while also reporting superior performance in targeting ADs .…”
