Diffuse Idiopathic Skeletal Hyperostosis of Cervical Spine with Dysphagia—Molecular and Clinical Aspects

Abstract: Diffuse idiopathic skeletal hyperostosis (DISH) is a condition characterized by the calcification and ossification of the ligaments of the cervical spine; in some cases, it may result in dysphagia. The condition is more common in men over 50 years of age with metabolic disorders, and it is often asymptomatic and not a major issue for patients. The etiology of DISH is poorly understood, and known genetic factors indicate multiple signal pathways and multigene inheritance. In this review, we discuss the epidemio… Show more

“…It is thought that DISH pathogenesis is polygenic and influenced by the interaction of many gene variants and environmental factors. The DISH phenotype consists of potential disturbances in various genes with different chromosomal localizations and expressions [ 6 ].…”

“…However, osteophytes in the cervical vertebrae are commonly asymptomatic, which leads to a delay in diagnosis [ 2 ]. Dysphagia is a complication presenting in 0.2%-28% of patients with DISH syndrome [ 6 ]. The presence of osteophytes at C3-C7 anteriorly compress, and if large enough, displace the oesophagus, leading to dysphagia, our patient’s presenting complaint [ 7 ].…”
