Diffuse choroidal haemangioma in Sturge–Weber syndrome treated with photodynamic therapy under general anaesthesia

Abstract: In our patient PDT with verteporfin effectively resolved the exudative retinal detachment associated with a diffuse choroidal haemangioma. Resolution of subretinal fluid occurred over several months without retreatment. We noted no side effects of the combination PDT and general anaesthesia, nor did we encounter ocular side effects of the treatment.

“…The most frequent treatment modality for diffuse choroidal hemangioma is external beam radiotherapy, with photons in 31 cases 73,74,81,113,138,148,162 and with protons in 21 cases. 47,81,221 The third most common method reported in the literature is PDT, which was used in 10 cases, where 7 cases were followed up after at least 6 months, 11,39,88,126,179,203 and 3 cases in 3 studies were followed up in less than 6 months. 9,18,203 In all cases treated with PDT, standard protocol with verteporfin was used, and the results are promising.Visual acuity increased, exudation resolved, and tumor size regressed.…”

“…Huiskamp et al [13] reported PDT according to the standard protocol under general anesthesia for exudative retinal detachment associated with diffuse choroidal hemangioma in a 12-year-old girl with Sturge-Weber syndrome. Subretinal fluid resolved completely over a period of 5 months and VA increased from 20/400 to 20/50.…”

“…The tumoricidal activity of PDT is caused by direct cell damage and microvascular injury 24. PDT has been used for the management of various intraocular tumors including circumscribed58 and diffuse choroidal hemangioma,59 choroidal melanoma,10–12 retinal hemangioblastoma,1316 retinal vasoproliferative tumor,1718 and retinal astrocytoma 1921. We describe the development of transient tumor vascular hyperpermeability with increased intraretinal/subretinal fluid following PDT of three different types of intraocular tumors (retinal hemangioblastoma, retinal astrocytoma, amelanotic choroidal melanoma).…”
