“…In addition, we proposed the benchmark operations based on the difficulty classification evaluated by the difficulty index. Several difficulty indexes, consisting of patient characteristics, anatomy, and tumor characteristics, have been used to classify the difficulty of several laparoscopic procedures [5][6][7][8]. Operative time has been reported to be a useful factor reflecting technical difficulty of laparoscopic procedures [15,16], therefore we established a grading system to predict the difficulty of LDN based on multivariable analyses investigating preoperative donor factors associated with prolonged operative time.…”

“…Using the difficulty index of 0-8, indexes 0 and 8 were defined as the easiest and the most difficult case. Difficulty index was further categorized into three levels as the difficulty classification: low difficulty (0-2), intermediate difficulty (3)(4)(5), and high difficulty (6)(7)(8).…”

“…The technique for minimally invasive distal pancreatectomy is indeed less demanding than the one required for minimally invasive pancreatoduodenectomy, so that virtually all pancreatic pancreatic surgeons, and most general surgeons, can perform this procedure safely in the absence of hostile anatomy and/or advanced tumor stage. However, minimally invasive distal pancreatectomy is a quite rare operation, even at high volume centers [24] , requiring careful patient selection [25] and the ability to fully master minimally invasive techniques [26] . Patient selection is required to avoid either unnecessary procedures in patients with benign lesions with limited risk of malignant degeneration [27] , or to plan the most appropriate therapeutic strategy for patients with pancreatic cancer [28] .…”

“…higher score in radical pancreatectomy for pancreatic cancer or spleen‐vessel‐preserving LDP), tumor close to major vessels including root of splenic artery or portal vein, pancreatic resection line (i.e. higher score at the level of portal vein requiring tunneling procedure), tumor extension to peripancreatic tissue, and left side portal hypertension . The highest difficulty score is given to radical LDP for pancreatic cancer located near the root of the splenic artery, obstructing the splenic vein leading to left‐side portal hypertension and invading peripancreatic tissue.…”
