Differential diagnosis of a lateral cervical cyst and solitary cystic lymph node metastasis of occult thyroid papillary carcinoma

Abstract: We present a case of solitary cystic lymph node metastasis from an occult thyroid papillary carcinoma mimicking a lateral cervical cyst, and a case of thyroid papillary carcinoma with a true lateral cervical cyst. Based on a comparison of the radiographical findings of the two cases, differential diagnosis between cystic lymph node metastasis of papillary carcinoma and branchial cyst is discussed.

“…Papillary thyroid carcinoma is characterized by slow growth and indolent biological behavior, but with frequent cervical lymph node metastases. By the time of diagnosis Papillary thyroid carcinoma has already spread to cervical lymph nodes in 20% -50% of patients [3,4]. Wallace and Betsill described that this cervical lymph node metastasis might undergo liquefaction and cystic transformation [5].…”

“…Cystic masses of the neck can also be lymph node metastases from and head and neck tumors [4]. Thyroid cysts are common but thyroid cancers presenting itself as a cystic mass may be due to metastasis to the cervical lymph nodes or as a primary true cystic mass without palpable lesion in the thyroid gland [5].…”

“…Due to the increasing incidence of oropharyngeal carcinoma, cystic masses of the neck can also be metastases from an oropharyngeal or tonsillar tumour [4]. Rarely, PTC is presented as a cystic neck mass without palpable lesion in the thyroid gland [5].…”
