One of the key principles of the Ottawa Charta is participation. Community health interventions should involve the community, i.e., residents and local actors. In Hamburg, we tried to put this principle into practice during a project initiated by the municipal health service in a disadvantaged quarter with about 3000 inhabitants. The contribution starts with an account of the meaning of participation in health promotion (1) and gives an overview of participation methods in general (2). The next part contains a short account of our project (3). Five examples are presented in part four: a survey with key actors and two approaches to listening to residents’ perceptions of capacity building in the neighborhood. Another survey of residents was meant to explore the use of health promotion offers and the preparedness to get involved. The last example is the round table of local actors as a sort of steering committee for health promotion interventions (4). Key findings of the examples are: Surveys do result in valuable information for the local actors. The expressed willingness to participate is larger than the actual participation. Opportunities to participate (in surveys and at the round table) are welcome by local actors. In the last section we discuss the advantages, problems and some illusions concerning participation in health promotion (5).

Zusammenfassung Die Ottawa-Charta von 1986 schuf ein neues Paradigma der Gesundheitsförderung, in dem Konzepte wie intersektorale Politik, Determinanten der Gesundheit und Vermitteln und Vernetzen eine wichtige Rolle spielen. In diesem Rahmen entstand die Forderung, dass Gesundheitsberichterstattung (GBE) in einer „integrierten“ Form zu erbringen sei, also Daten aus ganz verschiedenen Politiksektoren enthalten sollte, in denen Determinanten der Gesundheit zu finden sind. In diesem Beitrag soll der Frage nachgegangen werden, ob es im deutschsprachigen Raum auf kommunaler und Landesebene explizite Konzepte und eine öffentlich kommunizierte Praxis integrierter GBE gibt. Dazu wurde eine explorative Internetrecherche durchgeführt, ergänzt um E‑Mail-Anfragen bei Schlüsselpersonen der Projekte. Es wurden 8 Projekte identifiziert: 4 davon repräsentieren kommunale bzw. städtische integrierte Berichterstattung; in Nordrhein-Westfalen gibt es 2 Initiativen von Landesbehörden, die sich auf die kommunale Berichterstattung beziehen; in Berlin und im Burgenland (Österreich) geht es um integrierte Berichterstattung auf der Bundeslandebene. Die Projekte werden in stark komprimierter Form vorgestellt. Hauptlimitationen der explorativen Recherche sind die Beschränkung auf explizit als „integriert“ bezeichnete Berichterstattung und die eng eingeschränkten Suchbegriffe. Ein positiver Befund ist, dass es einige gute Indikatorenkonzepte, gesetzliche Rahmenbedingungen und förderliche Faktoren, wie z. B. wissenschaftliche Unterstützung, gibt. An der konkreten Umsetzung integrierter GBE mangelt es aber noch. Mögliche Hürden sind dabei die Komplexität der Programme, fehlende finanzielle, personelle und fachliche Ressourcen, organisatorische und methodische Probleme sowie Ängste und Vorurteile. In verschiedenen Beispielen konnten diese aber überwunden werden. Der Leitfaden Gute Praxis Gesundheitsberichterstattung stützt – explizit, aber etwas zu versteckt – die Forderung nach integrierter Berichterstattung.

Health-Related Disparities among Migrant Children at School Entry in Germany. How does the Definition of Migration Status Matter?

Background: Migration background is known to be an important risk factor for a number of medical outcomes. Still, relatively little is known about the epidemiologic relevance of different definitions of migration status. Methods: Data from 5250 school entry examinations spanning three consecutive years (2015–2017) were gathered from the Public Health Department in Halle, Germany. Data were stratified according to six different migration statuses and evaluated for differences in health service utilization and developmental outcomes. Results: Compared to non-migrant children, migrant children have a lower utilization of preventative services, and higher frequencies of developmental delays. Children with first-generation migration background consistently show results worse than all others, while children with one-sided second-generation migration background show results similar to those of their non-migrant peers. These findings are not substantially altered by adjustment for social status. Conclusions: Children with first-generation migration background should receive special attention in school entry examinations, since they constitute a group with consistently higher health risks compared to other groups of preschoolers.
