A study were performed to estimation the concentrations of the hormones (T3, T4 and TSH) , thyroglobulin(Tg) and thyroid peroxidase (TPO) aut antibodies , and the concentration of HLA-DR alleles in patients with thyroid diseases in Babylon province .The study reveals that the T4 levels were significantly increased (p< 0.05) in hyperthyroidism patients compared with other groups and healthy subject , also there is a significant increased (p< 0.05) in TSH level in hypothyroidism group compared with other groups; furthermore , the concentration of TPO auto-antibody show highly significant elevation (p<0.05) in hypothyroidism and hyperthyroidism patients compared with other group and healthy individuals. Meantime , there is significant differences (p<0.05) in TG auto-antibody between hyperthyroidism patients and other groups ;while there were significantly increased (p<0.05) in frequency of HLA-DR alleles in all groups of patients compared with healthy subjects , furthermore , the results reveals increased ratio of disease in females compared with males , also the ratio of thyroiditis patients were higher in second age categories compared with first and third age categories .

Autoimmune thyroid disease (AITD) is the most common organ-specific autoimmune disorder. AITD development occurs due to loss of immune tolerance and reactivity to thyroid autoantigens: thyroid peroxidase (TPO), thyroglobulin (TG) and thyroid stimulating hormone receptor (TSHR). This leads to infiltration of the gland by T cells and B cells that produce antibodies specific for clinical manifestations of hyperthyroidism in Graves' disease (GD) and chronic autoimmune thyroiditis (cAIT). In addition, T cells in Hashimoto's thyroiditis induce apoptosis in thyroid follicular cells, leading ultimately to the destruction of the gland. Cytokines are involved in the pathogenesis of thyroid diseases working in both the immune system and directly targeting the thyroid follicular cells. They are involved in the induction and effector phase of the immune response and inflammation, playing a key role in the pathogenesis of autoimmune thyroid disease. The presence of multiple cytokines has been demonstrated: IL-1a, IL-1b, IL-2, IL-4 , IL-6, IL-8, IL-10, IL-12, IL-13, IL-14, TNF-a and IFN-g within the inflammatory cells and thyroid follicular cells. Finally, cytokines derived from T cells can directly damage thyroid cells, leading to functional disorders and may also stimulate the production of nitric oxide (NO) and prostaglandin (PG), thus increasing the inflammatory response in AITD. Immunological mechanisms involved in the pathogenesis of AITD are strongly related to each other, but differences in the image of cAIT and GD phenotype are possibly due to a different type of immune response observed in these two counteracting clinical thyroid diseases. Selenium supplementation in people with Hashimoto's thyroiditis might reduce antibody levels and result in a decreased dosage of levothyroxine (LT₄) and may provide other beneficial effects (e.g. on mood and health-related quality of life). The aim of our systematic review was to assess the effects of selenium supplementation on Hashimoto's thyroiditis. We searched The Cochrane Library, MEDLINE, EMBASE and Web of Science for randomized controlled trials. Study selection, data extraction, assessment of risk of bias and analyses were carried out by two independent review authors. We assessed the quality of the evidence of included studies using GRADE. Four studies rated at unclear to high risk of bias comprising 463 participants were included. One study at high risk of bias showed statistically significant improvement in subjective well-being with sodium selenite 200 μg plus titrated LT₄ compared with placebo plus titrated LT₄ (RR 4.67, 95% CI 1.61-13.50). Selenomethionine 200 μg as a single treatment or combined with LT₄ reduced the serum levels of anti-thyroid peroxidase antibodies compared with placebo (or placebo plus LT₄) in three studies (p < 0.001). Although the changes from baseline were statistically significant in these three studies, their clinical relevance is unclear. In conclusion, the results of these four studies, assessed at unclear to high risk of bias, show that evidence to support or refute the efficacy of selenium supplementation in people with Hashimoto's thyroiditis is incomplete and not reliable to help inform clinical decision making.
