Diazepam in Emulsion Form for Intravenous Usage

Abstract: Since aqueous solutions of diazepam were introduced into clinical practice, the problem has arisen of local vascular side effects after intravenous injection. A new preparation of diazepam as a lipid emulsion is presented. It was prepared by Vitrum and shows many similarities to Intralipid. A clinical trial was designed to study the incidence of pain in connection with the intravenous injection of diazepam and the incidence of subsequent thrombophlebitis. The preparation was used as a sedative to patients befo… Show more

“…Clinically, the use of diazepam in the emulsion vehicle has been investigated on a small scale (250 patient doses) by von Dardel, Mebius & Mossberg (1976). No significant difference could be demonstrated in effect when compared with Valium@ injection but there was a marked reduction of pain at the site of injection.…”

“…In a clinical investigation, VON DARDEL et al (1976) found a highly significant reduction in the incidence of adverse local vascular effects after intravenous injection of diazepam emulsion, but the pharmacokinetic properties were not investigated in this trial.…”

“…Aqueous solutions of diazepam are known to cause local damage to the vein after intravenous application (LANGDON 1973, VON DARDEL et al 1976). This can be partly reduced by using macrogoli ricinoleas (Cremophor EL) instead of propylene glycol as solubilizer, but this alternative enhances the risk of allergic manifestations.…”

“…Diazepam (Von Dardel et al 1976, erythromycin (Marlin et al 1983) and clarithromycin (Lovell et al 1994;Cannon et al 1995) have also been known to cause pain on injection. There may be several possible reasons for this, including precipitation of drug at the site of injection, contamination of the solution with foreign particles (Cannon et al 1995), and the hypo-or hyperosmolarity of the drug solution (Klement & Arndt 1991).…”

“…The oil-in-water emulsion system has been shown to be eOE ective in reducing the pain response on injection by encapsulating drugs, such as diazepam and propranolol (Von Dardel et al 1976Doenicke et al 1996). Furthermore, the encapsulation of poorly water-soluble drugs in the emulsion system can provide a formulation with improved drug solubility (Strickley & Anderson 1993;Park & Kim 1999;Perkins et al 2000).…”
