Diagnostic Value of Peritumoral Minimum Apparent Diffusion Coefficient for Differentiation of Glioblastoma Multiforme From Solitary Metastatic Lesions

Abstract: Although the characteristics of solitary metastasis and glioblastoma multiforme may be similar on conventional MRI, DWI can offer diagnostic information to distinguish between the tumors.

“…The present definition of minimum ADC is the lowest value selected from multiple ROIs drawn manually along the border of the lesion on each consecutive tumor-containing slices, including the whole tumor volume, not the lowest value corresponding to a pixel in one ROI surrounding the mass. The regions with minimum ADCs have been suggested to reflect the highest tumor cell density or the most proliferative portion of the tumor (20).…”

“…11,12 The third type is focused on 3 locations: the enhancing part of the tumor, regions with peritumoral T2 prolongation, and the necrotic areas within the tumor. 7 With regard to the enhancing part of the tumor, DTI and perfusion metrics showed inconsistent conclusions, with some authors believing there was no difference between the 2 types of tumors, 10,13,16 contrary to the others. 6,8,14,15 As for the peritumoral T2 prolongation, the results were also controversial.…”

“…2,3 Therefore, it is thought to be very difficult to distinguish between these 2 types of tumors with routine MR imaging alone. [4][5][6] Many studies have focused on other advanced MR imaging modalities, such as DTI, 3,5,7,8 DWI, 9,10 PWI, 6,[11][12][13][14][15][16] and MR spectroscopy, 17 to explore the differentiating characteristics.…”

“…Specifically, although peritumoral oedema is common in both metastases and primary brain malignancies, the ADC values in the peritumoral oedema of metastases have been shown to be higher than those of primary brain malignancies. It is believed that this difference is owing to the more infiltrative nature of primary malignancies, which results in an "infiltrative oedema" as opposed to simple vasogenic oedema [67].…”
