Diagnostic value of interferon-γ release assay in HIV-infected individuals complicated with active tuberculosis: a systematic review and meta-analysis

Abstract: Objective Although the interferon-γ release assay (IGRA) has become a common diagnostic method for tuberculosis, its value in the diagnosis of tuberculosis in human immunodeficiency virus (HIV) seropositive patients remains controversial. Therefore, this systematically reviews the data for exploring the diagnostic value of IGRA in HIV-infected individuals complicated with active tuberculosis, aiming to provide a clinical basis for future clinical diagnosis of the disease. Methods Relevan… Show more

“…A challenging issue in HIV-TB coinfection is the atypical presentation of TB [ 40 ]. While the conventional sputum-based detection of TB is of limited scope in these patients and often leads to false negativity [ 41 ], interferon gamma (IFN-γ)-based assays, though specific [ 42 ], are more expensive. Thus, several studies have applied transcriptomics, particularly the altered or differentially expressed protein-coding genes between HIV and HIV-TB patients, to identify biological markers for TB in HIV patients [ 43 , 44 ].…”

“…показали эффективность лабораторного метода Т-СПОТ.ТБ в выявлении ЛТИ у взрослых пациентов в Самарской области [26]. Высокая эффективность IGRA-тестов у ВИЧ-инфицированных лиц отмечена по результатам метаанализа, проведенного Y. Ma et al [27]. В работе A.C. Pettit et al [28] показано, что методика ELISPOT (тест Т-СПОТ.ТБ) имеет более высокую положительную прогностическую значимость для этой категории пациентов.…”

“…However, this hypothesis was ruled out because of positive STN-PCR results for M. tuberculosis. Based on a meta-analysis study conducted by Ma, et al, (2021), IGRA is more sensitive in detecting TB in HIV-positive patients compared to TST, which can cause falsenegative results. However, in this patient, the TST result was positive and the IGRA result was negative.…”
