Head and neck cancers are amongst the commonest malignancies, accounting for approximately 20% of the cancer burden in India. The major risk factors are tobacco chewing, smoking and alcohol consumption, which from 2002-2006 at Patna Medical College and Hospital, a tertiary care hospital drawing patients from the entire Bihar state, the 3rd most populous state of India with the majority of the population residing in rural areas. Incidence rates based on sex, age, site of lesion, including age standardized incidence rates for males and females, with mean age of presentation, distribution of histological variants and year wise trend were calculated. Out of 455 head and neck neoplasias, 241 were benign while 214 were malignant. The most common age group for all malignant biopsies was 7th decade for males and the 5th decade for females. Malignant cases were commoner 2 ) test. The crude rate and age standardized incidence rate was 0.05 and 0.06 per 100,000 population respectively. Squamous cell carcinoma (SCC) contributed about 96% of all cases, with grade I being the most common. Larynx was the most common site for malignancy, the supraglottic region being its most commonly affected sub-site. This observed incidence patterns in the region are a reminder of widespread unawareness, low healthcare utilization with virtually non-existent cancer programs. It also underlines the need to advocate for reliable cost-effective programs to create awareness, for early detection and plan appropriate management strategies. There is a compelling demand for a cancer registry in this region as well as proper implementation of preventive measures to combat this growing threat of cancer, many of whose risk factors are preventable.

Head and neck squamous cell carcinoma is one of the most aggressive tumours and is typically diagnosed too late. Late diagnosis requires an urgent decision on an effective therapy. An individualized test of chemosensitivity should quickly indicate the suitability of chemotherapy and radiotherapy. No ex vivo chemosensitivity assessment developed thus far has become a part of general clinical practice. Therefore, we attempted to explore the new technique of coherence-controlled holographic microscopy to investigate the motility and growth of live cells from a head and neck squamous cell carcinoma biopsy. We expected to reveal behavioural patterns characteristic for malignant cells that can be used to imrove future predictive evaluation of chemotherapy. We managed to cultivate primary SACR2 carcinoma cells from head and neck squamous cell carcinoma biopsy verified through histopathology. The cells grew as a cohesive sheet of suspected carcinoma origin, and western blots showed positivity for the tumour marker p63 confirming cancerous origin. Unlike the roundish colonies of the established FaDu carcinoma cell line, the SACR2 cells formed irregularly shaped colonies, eliciting the impression of the collective invasion of carcinoma cells. Time-lapse recordings of the cohesive sheet activity revealed the rapid migration and high plasticity of these epithelial-like cells. Individual cells frequently abandoned the swiftly migrating crowd by moving aside and crawling faster. The increasing mass of fast migrating epithelial-like cells before and after mitosis confirmed the continuation of the cell cycle. In immunofluorescence, analogously shaped cells expressed the p63 tumour marker, considered proof of their origin from a carcinoma. These behavioural traits indicate the feasible identification of carcinoma cells in culture according to the proposed concept of the carcinoma cell dynamic phenotype. If further developed, this approach could later serve in a new functional online analysis of reactions of carcinoma cells to therapy. Such efforts conform to current trends in precision medicine.

Objective: Bone marrow involvement (BMI) is a significant component of staging of Hodgkin's lymphoma (HL). Unilateral bone marrow biopsy (BMB) from dorsal iliac bone is the standard method to determine BMI. Positron emission tomography/computerized tomography (PET/CT) is recommended as a complimentary technique to determine BMI and to evaluate response to treatment. The aim of this study is to determine whether PET/CT can replace BMB to detect BMI in patients with HL. Methods: A total of 159 patients diagnosed as having HL were evaluated retrospectively. One hundred and four patients who met the criteria were included in the study. BMB and PET/CT were performed on all patients during initial staging. Results: Of the 104 patients, 44 (42.3%) and 17 (16.3%) had BMI in PET/CT and BMB respectively. All patients who had BMI in BMB also had involvement in PET/CT. BMB did not detect BMI in 27 patients who had BMI in PET/CT. All 27 patients had partial or complete remission on PET/CT performed at the end of treatment. This finding was regarded as a relative indicator for BMI. PET/CT had a sensitivity and negative predictive value of 100%, specificity of 68.9% and accuracy rate of 74% to determine BMI. Conclusion: We observed that PET/CT and BMB were compatible to determine BMI in patients diagnosed with HL. Unilateral BMB may result false negative especially in cases with focal involvement. Therefore, PET/CT should be considered as a complimentary technique to determine BMI. Staging should be reevaluated in patients who have BMI in PET/CT and a negative BMB treatment should be planned accordingly.
