Diagnostic pitfalls of peritoneal washing cytology and the role of cell blocks in their diagnosis

Abstract: Mesothelial cell hyperplasia, collagen balls, endometriosis, and endosalpingiosis are diagnostic pitfalls on peritoneal washing cytology in women who present with gynecologic lesions. Over an 8-month period, the peritoneal washings from 10 patients undergoing gynecologic surgery for presumed malignancy showed unusual cytologic findings, several of which posed diagnostic difficulties. The washings from four patients with ovarian carcinomas were cellular and contained clusters and strips of cells with cytologic … Show more

“…Selvaggi [28] reported that collagen balls – in addition to mesothelial cell hyperplasia, endometriosis, and endosalpingiosis – are diagnostic pitfalls on PW cytopathology in women who present with gynecologic tumors. On the other hand, preparation of cell blocks from PWs was of value in the work-up and management of patients who present with cytopathologic mimickers of malignancy on PW cytopathology [28].…”

“…10,13 Psammoma bodies are mineralized concretions most commonly associated with the cytology of ovarian epithelial tumors. 11 Globules were not mineralized in this case. Although rare, yolk sac carcinoma should be included in the differential diagnosis for ascites and carcinomatosis in rats.…”

“…12 Differentials for the acellular globules surrounded by cells observed in the abdominal fluid cytology include collagen balls and psammoma bodies. 11 Collagen balls (serosal balls) are associated with benign mesothelial cells and composed of type I and III collagen as documented by immunostaining. 10,13 Collagen balls are Masson's trichrome positive and weakly PAS positive.…”

“…The limitations of PWC examination are the possibility of false positivity in benign diseases and false negativity in the early stage of disease. Although positive PWC does not seem to change the stage of the disease in the latest guidelines, it is a poor prognostic factor and effective in determining the intensity of the treatment (Selvaggi et al, 2003). In this study, CEA, CA 125 and CA 19-9 in peritoneal washing fluid were evaluated in patients operated due to a gynaecologic cancer.…”
