Laparoscopic chromopertubation is considered "gold standard" for checking the tubal patency. Foley catheter is frequently used for blue dye during chromopertubation. Complications associated with the intra-uterine use of Foley catheter are infrequent. The mean normal capacity of the uterine cavity is about 9 ml, and an inflation of up to 30 ml (e. g. during thermal balloon ablation procedures) is considered safe. We report a uterine rupture in a 36-year-old woman undergoing laparoscopic chromopertubation due to primary infertility. Thirteen years ago, the patient had three consecutive laparotomies because of appendicitis, peritonitis and retroperitoneal abscess. For the present laparoscopy, the Foley catheter (Nelaton, charier 10, balloon 5 ml) was used. The first blocking of the balloon with 3.5 ml saline was insufficient; however after inflating with 5 ml, a rupture of the uterine fundus occurred. The balloon remained intact and both tubes appeared patent. The myometrium was sutured and the postoperative course was uneventful. We presume that-in the present case-the expansive capacity of the uterine wall may have been reduced after the series of severe pelvic inflammations. Nevertheless, if using a Foley catheter for the chromopertubation, the optimal pressure for its intrauterine fixation needs still to be determined.

Objective. Tubal pathology accounts for a third of infertility cases of which about 80 percent affects the ampulla and distal tube and 10 -25 percent the proximal tube. Infertile patients with tubal pathology have two options for treatment, in vitro fertilization or tubal reconstructive microsurgery. The present study aims to perform a literature review to give a comprehensive knowledge of the role of salpingoscopy/ falloposcopy in the assessment of tubal pathology in infertile couples and its impact on clinical practice. Materials and Methods.A review of various articles on the technique of salpingoscopy/ falloposcopy and its clinical use in infertile patients was undertaken by searching databases like Pubmed, Scopus, Medline, Cochrane database, Embase, Web of science, Science direct, etc. The studies describing the methods and clinical scope of salpingoscopy and falloposcopy were included in the present study.Results. The method of salpingoscopy/ falloposcopy and its role in the management of infertile patients has been studied in various aspects. The technique, the assessment and classification of tubal pathology, and the clinical impact on the management of the infertile couple has been reported as per actual literature data.Conclusions. Salpingoscopy and falloposcopy are two important methods to assess the tubal mucosal surface. The conventional methods of examining the tubes by laparoscopy and HSG are inadequate for the study of tubal pathology. An endoscopic examination of the fallopian tubes along with HSG and laparoscopic evaluation of the pelvis will give complete information about the fallopian tubes.

Purpose of reviewThe last decade has witnessed a radical change in the field of reproductive surgery. The increasing success of in-vitro fertilization (IVF) has caused a huge shift in emphasis with many downstream consequences. This review outlines the changes and provides insight into the future of reproductive surgery. Recent findingsWith compelling evidence that IVF overcomes the detrimental effects of endometriosis on infertility and with two new oral medications available for management of endometriosis, momentum is shifting towards nonsurgical management of endometriosis. There is increasing recognition that except for submucous myomas, other myomas are unlikely to affect fertility and miscarriage. This, in addition to many emerging alternative modalities for management of myomas (oral GnRH antagonists, radiofrequency ablation), is likely to further decrease classic myomectomies but provide alternative, less invasive options. Caesarean scar defects have been recognized as having significant reproductive consequences and surgical management has become the standard of care. Fallopian tubes are now implicated in development of ovarian cancer, and as a result, salpingectomies are being performed in lieu of tubal ligations. Tubal anastomosis will soon become a historical surgery. Division of uterine septum remains controversial, and a clear answer will remain elusive. Uterine transplant is the single most significant advance in reproductive surgery in the past century. SummaryReproductive surgery is evolving with the times. Although some surgical techniques will become historical, others will become mainstream.

Abstract: Falloposcopy surgeon provides good opportunities for the diagnosis and treatment of intralumen lesions. Significant increase in the frequency of uterine pregnancy in the case of an additional step - TFTD during salpingolysis, salpingostomy in patients with tubal factor infertility.
